Lonafarnib falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Ipriflavone interacts with.

Ipriflavone is thought to competitively inhibit cytochrome P450 2C9 (CYP2C9). Theoretically, concurrent use of ipriflavone and drugs metabolized by these enzymes might result in decreased drug elimination, increased serum levels, and potential toxicity. Some drugs metabolized by CYP2C9 include amitriptyline (Elavil), diazepam (Valium), estradiol (Estrace), tacrine (Cognex), verapamil (Calan), warfarin (Coumadin), zileuton (Zyflo), and others.

Here's the gist for Zokinvy (lonafarnib). Ipriflavone may gently block an enzyme called CYP2C9, which is one of the pathways your body uses to clear certain medicines. If that pathway slows down, lonafarnib could build up a bit more than expected, which might mean more of its usual side effects like nausea, vomiting, diarrhea, poor appetite, or tiredness.

I want to be honest about the strength of this one: it's theoretical, based only on lab studies, not real patient cases. So it may never cause a real-world problem. Still, lonafarnib is a sensitive drug, so don't add ipriflavone on your own. Run it by your pharmacist or doctor first so they can check your specific dose and watch for any issues.

Ipriflavone is a synthetic isoflavone studied mainly for bone health and osteoporosis, but high-quality research has not shown that it reliably prevents fractures or builds strong bone. Because it can lower white blood cell counts in some people and its long-term safety is not well established, talk with your pharmacist or doctor before using it.

What Is Lonafarnib Used For?

Brand name: Zokinvy

Zokinvy (lonafarnib) is approved for patients 12 months of age and older who have a body surface area of 0.39 m² or above. It is used to reduce the risk of mortality in Hutchinson-Gilford Progeria Syndrome (HGPS) — a rare, severe premature-aging disorder.

Zokinvy is also used to treat processing-deficient Progeroid Laminopathies — a related group of rare genetic conditions — specifically those caused by a heterozygous LMNA mutation with progerin-like protein buildup, or by homozygous or compound heterozygous ZMPSTE24 mutations. It is not indicated for other Progeroid Syndromes or processing-proficient Progeroid Laminopathies.
