Imipramine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Ipriflavone interacts with.

Ipriflavone is thought to competitively inhibit cytochrome P450 1A2 (CYP1A2). Theoretically, concurrent use of ipriflavone and drugs metabolized by these enzymes might result in decreased drug elimination, increased serum levels, and potential toxicity. Some drugs metabolized by CYP1A2 include clozapine (Clozaril), cyclobenzaprine (Flexeril), fluvoxamine (Luvox), haloperidol (Haldol), imipramine (Tofranil), mexiletine (Mexitil), olanzapine (Zyprexa), Pentazocine (Talwin), propranolol (Inderal), tacrine (Cognex), theophylline (Slo-bid, Theo-Dur, others), zileuton (Zyflo), Zolmitriptan (Zomig), and others.

Here's the gist: your body uses an enzyme called CYP1A2 to help clear imipramine out. Ipriflavone may slow that same enzyme down, so in theory imipramine could clear more slowly and build up to higher levels in your blood. Imipramine isn't a prodrug, so more drug hanging around generally means stronger effects and more side effects, things like dry mouth, constipation, blurry vision, dizziness when you stand, drowsiness, or a faster heartbeat.

I'll be honest though, this concern comes only from lab-type studies, not real patients, so it's theoretical right now. Still, since tricyclics like imipramine can be touchy at higher levels, it's worth a quick chat with your pharmacist or doctor before combining the two.

Ipriflavone is thought to competitively inhibit cytochrome P450 2C9 (CYP2C9). Theoretically, concurrent use of ipriflavone and drugs metabolized by these enzymes might result in decreased drug elimination, increased serum levels, and potential toxicity. Some drugs metabolized by CYP2C9 include amitriptyline (Elavil), diazepam (Valium), estradiol (Estrace), tacrine (Cognex), verapamil (Calan), warfarin (Coumadin), zileuton (Zyflo), and others.

Here's the short version: ipriflavone may gently block a liver enzyme called CYP2C9, which is one of the pathways your body uses to clear imipramine. If that pathway slows down, more imipramine could linger in your bloodstream. That can mean a stronger drug effect and more of imipramine's usual side effects, things like drowsiness, dry mouth, constipation, blurred vision, dizziness on standing, or a faster heartbeat.

I want to keep this in perspective, though. This concern comes only from lab studies, not from real people, so we don't actually know how much it matters day to day. Still, since tricyclics can be touchy at higher levels, it's worth a quick chat with your pharmacist or doctor before combining them rather than guessing.

Ipriflavone is a synthetic isoflavone studied mainly for bone health and osteoporosis, but high-quality research has not shown that it reliably prevents fractures or builds strong bone. Because it can lower white blood cell counts in some people and its long-term safety is not well established, talk with your pharmacist or doctor before using it.

What Is Imipramine Used For?

Brand names: Tofranil, Tofranil PM

Imipramine is used to relieve symptoms of depression. Both Imipramine Hydrochloride tablets and Imipramine Pamoate capsules are approved for this use. Endogenous depression — the kind that seems to arise from within rather than a clear outside cause — tends to respond especially well. It may take one to three weeks before you notice the full benefit.

Imipramine Hydrochloride tablets are also approved as a short-term, add-on treatment to help reduce nighttime bedwetting (nocturnal enuresis) in children aged 6 and older, after a doctor has ruled out physical causes.
