Clomipramine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Ipriflavone interacts with.

Ipriflavone is thought to competitively inhibit cytochrome P450 1A2 (CYP1A2). Theoretically, concurrent use of ipriflavone and drugs metabolized by these enzymes might result in decreased drug elimination, increased serum levels, and potential toxicity. Some drugs metabolized by CYP1A2 include clozapine (Clozaril), cyclobenzaprine (Flexeril), fluvoxamine (Luvox), haloperidol (Haldol), imipramine (Tofranil), mexiletine (Mexitil), olanzapine (Zyprexa), Pentazocine (Talwin), propranolol (Inderal), tacrine (Cognex), theophylline (Slo-bid, Theo-Dur, others), zileuton (Zyflo), Zolmitriptan (Zomig), and others.

Here's the gist for clomipramine (Anafranil). Your liver uses an enzyme called CYP1A2 to help clear this medication out of your system. Ipriflavone is thought to slow that same enzyme down, so in theory clomipramine could clear more slowly and build up to higher levels in your blood.

If that happened, you'd mostly notice more of clomipramine's usual side effects, things like drowsiness, dry mouth, constipation, blurry vision, dizziness when standing, or a faster heartbeat. I want to be honest, though: this concern comes only from lab-type studies, not real patient cases, so it's more of a theoretical caution than a proven problem. Still, since clomipramine can affect heart rhythm at higher levels, it's worth a quick chat with your pharmacist or doctor before combining the two.

Ipriflavone is thought to competitively inhibit cytochrome P450 2C9 (CYP2C9). Theoretically, concurrent use of ipriflavone and drugs metabolized by these enzymes might result in decreased drug elimination, increased serum levels, and potential toxicity. Some drugs metabolized by CYP2C9 include amitriptyline (Elavil), diazepam (Valium), estradiol (Estrace), tacrine (Cognex), verapamil (Calan), warfarin (Coumadin), zileuton (Zyflo), and others.

Here's the gist for Anafranil (clomipramine): your liver uses a handful of enzymes to clear it, and one of them is CYP2C9. Ipriflavone is thought to gently block that enzyme, so in theory clomipramine could leave your body a little slower and build up to higher levels. Clomipramine is the active drug itself (not a prodrug), so higher levels mean stronger effects and a greater chance of side effects like drowsiness, dry mouth, constipation, blurry vision, dizziness on standing, or a faster heartbeat.

I want to be straight with you though: this concern comes only from lab and animal studies, not real patients, so it's more theoretical than proven. Still, since clomipramine can affect the heart, it's worth a quick chat with your pharmacist or doctor before adding ipriflavone.

Clomipramine — sold as Anafranil and under the generic name Clomipramine Hydrochloride — is approved to treat obsessions and compulsions in people with Obsessive-Compulsive Disorder (OCD). This includes adults and children/adolescents ages 10 to 17.

To qualify for treatment, the obsessions or compulsions must cause significant distress, take up a lot of time, or noticeably interfere with work, school, or social life.
