Carvedilol falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Ipriflavone interacts with.

Ipriflavone is thought to competitively inhibit cytochrome P450 2C9 (CYP2C9). Theoretically, concurrent use of ipriflavone and drugs metabolized by these enzymes might result in decreased drug elimination, increased serum levels, and potential toxicity. Some drugs metabolized by CYP2C9 include amitriptyline (Elavil), diazepam (Valium), estradiol (Estrace), tacrine (Cognex), verapamil (Calan), warfarin (Coumadin), zileuton (Zyflo), and others.

Here's the gist for carvedilol (Coreg). Your liver uses an enzyme called CYP2C9 to help clear this medicine. Ipriflavone may gently block that same enzyme, so in theory carvedilol could hang around longer and build up a bit in your blood. If that happens, you might notice more of carvedilol's usual effects, like a slower heart rate, lower blood pressure, dizziness, lightheadedness when you stand up, or feeling unusually tired.

I want to be honest about the strength of this, though. The concern comes only from lab studies, not from real-world reports in people, so it's more of a theoretical caution than something we see often. Still, it's worth a quick chat with your pharmacist or doctor before combining them, especially if you already run a low heart rate or blood pressure.

What Is Carvedilol Used For?

Carvedilol is used to treat three main conditions: mild-to-severe chronic heart failure, high blood pressure (hypertension), and reduced heart pumping function (left ventricular dysfunction) in people who have survived a heart attack. For heart failure, it is usually prescribed alongside other medications such as diuretics, ACE inhibitors, and digitalis to help people live longer and reduce the chance of hospital stays.

For people who have had a heart attack and whose heart is not pumping as well as it should, carvedilol is used to lower the risk of dying from heart-related causes. For high blood pressure, it can be used on its own or combined with other blood pressure medicines.
