Cladribine falls under IMMUNOSUPPRESSANTS — the group Ipriflavone interacts with.

Concurrent use of ipriflavone and immunosuppressant drugs might have additive effects on inhibiting immune function. Some patients taking ipriflavone experience subclinical lymphocytopenia; avoid concurrent use. Immunosuppressant drugs include azathioprine (Imuran), basiliximab (Simulect), cyclosporine (Neoral, Sandimmune), daclizumab (Zenapax), muromonab-CD3 (OKT3, Orthoclone OKT3), mycophenolate (CellCept), tacrolimus (FK506, Prograf), sirolimus (Rapamune), prednisone (Deltasone, Orasone), and other corticosteroids (glucocorticoids).

Here's the concern with pairing these two. Cladribine works by knocking down certain immune cells, especially lymphocytes, and lowering them is actually how it treats your MS. The catch is that ipriflavone has been shown to lower white blood cell counts too, sometimes dropping lymphocytes without any obvious symptoms. Stacking them together could push your immune cells down further than intended.

In real life, that may mean a higher chance of infections, fevers, or feeling run-down, since a deeper drop in lymphocytes leaves you more open to getting sick. This isn't about ipriflavone changing how cladribine is broken down, it's two things pulling your immune system in the same direction. Because cladribine already requires close blood monitoring, please check with your neurologist or pharmacist before adding ipriflavone.

What Is Cladribine Used For?

Brand names: Leustatin, Mavenclad

Cladribine tablets (brand name Mavenclad) are used to treat relapsing forms of multiple sclerosis (MS) in adults — specifically relapsing-remitting MS and active secondary progressive MS. Because of its significant safety profile, it is generally reserved for patients who haven't responded well to, or can't tolerate, other MS medicines. It is not recommended for clinically isolated syndrome (CIS), a first episode that may or may not lead to MS.

Cladribine injection (given intravenously) is a separate product used to treat active hairy cell leukemia — a rare blood cancer — in patients who have clinically significant anemia, low white blood cell counts, low platelets, or disease-related symptoms.
