Nortriptyline falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Ipriflavone interacts with.

Ipriflavone is thought to competitively inhibit cytochrome P450 1A2 (CYP1A2). Theoretically, concurrent use of ipriflavone and drugs metabolized by these enzymes might result in decreased drug elimination, increased serum levels, and potential toxicity. Some drugs metabolized by CYP1A2 include clozapine (Clozaril), cyclobenzaprine (Flexeril), fluvoxamine (Luvox), haloperidol (Haldol), imipramine (Tofranil), mexiletine (Mexitil), olanzapine (Zyprexa), Pentazocine (Talwin), propranolol (Inderal), tacrine (Cognex), theophylline (Slo-bid, Theo-Dur, others), zileuton (Zyflo), Zolmitriptan (Zomig), and others.

Here's the basic idea with these two together. Your liver uses an enzyme called CYP1A2 to help clear out nortriptyline. Ipriflavone may slow that enzyme down, so in theory nortriptyline could hang around longer and build up in your blood. That could mean stronger effects and more of the usual nortriptyline side effects, like drowsiness, dry mouth, constipation, blurred vision, dizziness when you stand up, or a faster heartbeat.

Now, the honest part: this concern comes from lab studies, not real people, so we don't truly know how big a deal it is day to day. Still, since tricyclics can affect heart rhythm, it's worth a quick chat with your pharmacist or doctor before combining them. Don't change anything on your own.
