Nortriptyline falls under TRICYCLIC ANTIDEPRESSANTS (TCAs) — the group Cowhage interacts with.

Theoretically, use of TCAs might reduce the levels and clinical effects of cowhage.
Cowhage contains levodopa. Use of TCAs might reduce the absorption of levodopa. Some case reports describe patients that developed hypertension and dyskinesia when taking both levodopa and TCAs.

Here's the situation if you're thinking about pairing cowhage with your nortriptyline (Pamelor). Cowhage is a natural source of L-dopa, the same active compound found in some Parkinson's medications. The concern works in two directions, though I want to stress this is theoretical, based on how these compounds behave rather than solid studies.

First, a tricyclic like nortriptyline may slow down how well your gut absorbs the L-dopa, so the cowhage might not work as well as expected. Second, a handful of older case reports describe people on both ending up with raised blood pressure and unusual involuntary movements (dyskinesia). Neither effect is guaranteed, but the combination is hard to predict. Before mixing these, please check with your doctor or pharmacist so they can weigh it for you.
