Phenelzine Sulfate falls under MONOAMINE OXIDASE INHIBITORS (MAOIs) — the group Cowhage interacts with.

Theoretically, concomitant use of cowhage and non-selective MAOIs might increase the risk of hypertensive crisis.
Cowhage contains levodopa. Use of levodopa with non-selective MAOIs might cause hypertensive crisis. However, this interaction has not been reported with MAO-B selective inhibitors such as selegiline.

Here's the concern with this pairing. Cowhage is a natural source of L-dopa, the same building block of dopamine used in some Parkinson's drugs. Phenelzine (Nardil) is a non-selective MAO inhibitor, meaning it blocks the enzyme your body normally uses to break down certain stimulating chemicals. When you add an L-dopa source like cowhage on top of that blockade, those compounds can pile up.

The worry is a hypertensive crisis, a sudden, dangerous spike in blood pressure that can bring on a pounding headache, chest pain, sweating, or a racing heart. This is rated as a theoretical interaction based on pharmacology, not a long list of reported cases, but because it's potentially serious I'd treat it cautiously. Please don't combine cowhage with Nardil without first checking with your doctor or pharmacist.

What Is Phenelzine Used For?

Brand name: Nardil

Phenelzine is used to treat depression — specifically the types described as atypical, nonendogenous, or neurotic depression. This often includes people who have a mix of anxiety and depression, or who have phobic or hypochondriacal features (strong fears about illness or physical symptoms).

It is generally not the first antidepressant a doctor will try. It is best suited for patients who have already tried other antidepressants without enough relief. There is less evidence supporting its use in severe depression with classic biological features (called endogenous depression).
