Brexpiprazole falls under ANTIPSYCHOTIC DRUGS — the group Cowhage interacts with.

Theoretically, use of cowhage might decrease the clinical effects of antipsychotic drugs.
Cowhage contains levodopa. Use of levodopa might counteract the antidopaminergic effects of antipsychotic medications.

Here's the gist of it. Brexpiprazole (Rexulti) calms down some of the dopamine signaling in your brain to help with schizophrenia or stubborn depression. Cowhage works in the opposite direction because it's a natural source of L-dopa, the same building block your body turns into dopamine. So in theory, the two could work against each other a bit, and you might notice Rexulti not doing its job as well.

If that happened, it could look like a return of symptoms the medicine was keeping in check, things like agitation, racing or disorganized thoughts, or slipping mood. To be fair, this concern is based on how these compounds behave on paper, not on solid studies in people. Still, because cowhage can pack a real L-dopa punch, please loop in your doctor or pharmacist before mixing them.

About Brexpiprazole

What Is Brexpiprazole Used For?

Brand name: Rexulti

Brexpiprazole tablets are approved for two uses in adults: as an add-on (adjunctive) treatment alongside antidepressants for major depressive disorder (MDD), and as a treatment for schizophrenia. These uses apply to both the generic brexpiprazole tablets and Rexulti.

Rexulti has one additional approved use: treating agitation associated with dementia due to Alzheimer's disease in adults. Rexulti is also approved for schizophrenia in adolescents ages 13 and older. Note that Rexulti is not approved for dementia-related psychosis without agitation from Alzheimer's disease, and it is not meant to be used on an as-needed basis for Alzheimer's-related agitation.
