Why this applies to Moclobemide

Moclobemide falls under MONOAMINE OXIDASE INHIBITORS (MAOIs) — the group Cowhage interacts with.

Theoretically, concomitant use of cowhage and non-selective MAOIs might increase the risk of hypertensive crisis.
Cowhage contains levodopa. Use of levodopa with non-selective MAOIs might cause hypertensive crisis. However, this interaction has not been reported with MAO-B selective inhibitors such as selegiline.

Here's the concern with pairing cowhage and moclobemide. Cowhage is a natural source of L-dopa, the same building block your body turns into dopamine. Moclobemide is an MAO inhibitor (it slows down an enzyme that breaks down dopamine and similar messengers). Put those two together and you can end up with too much of these stimulating chemicals hanging around, which can push blood pressure up sharply. That sudden spike, called a hypertensive crisis, can show up as a pounding headache, chest discomfort, a racing heart, or feeling flushed and sweaty.

I'll be honest: this is a theoretical warning based on how these drugs work, not a stack of real-world reports. Moclobemide is also more selective than the older MAOIs, which may lower the risk. Still, because the worst-case outcome is serious, please don't combine them without checking with your doctor or pharmacist first.
