Here's the gist of it. Clozapine works by calming down dopamine activity in the brain to ease the symptoms of schizophrenia. Cowhage is a natural source of L-dopa, which your body turns into more dopamine. So these two are basically pulling in opposite directions, and in theory cowhage could chip away at how well your clozapine controls your symptoms.

I want to be honest about the evidence here: this is a theoretical concern based on how these compounds work, not something proven in real-world studies with clozapine. Still, with a medication as important and carefully managed as clozapine, it's not worth gambling on. Please loop in your doctor or pharmacist before adding cowhage so nobody changes your dose blindly.

What Is Clozapine Used For?

Brand names: Clozaril, Versacloz

Clozapine is used for two specific situations. First, it is prescribed for people with severe schizophrenia who have already tried other antipsychotic medications and have not gotten adequate relief. It is reserved for this group because of its serious risks.

Second, clozapine is used to lower the chances of repeated suicidal behavior in people with schizophrenia or schizoaffective disorder who are considered to be at ongoing, chronic risk. It is not a first-choice medication — it is used when the need is serious and other options have fallen short.
