Clomipramine falls under TRICYCLIC ANTIDEPRESSANTS (TCAs) — the group Cowhage interacts with.

Theoretically, use of TCAs might reduce the levels and clinical effects of cowhage.
Cowhage contains levodopa. Use of TCAs might reduce the absorption of levodopa. Some case reports describe patients that developed hypertension and dyskinesia when taking both levodopa and TCAs.

Here's the deal with mixing cowhage and Clomipramine (Anafranil). Cowhage is basically a plant source of levodopa, the same active compound found in Parkinson's medicines. Clomipramine is a tricyclic antidepressant, and it isn't a prodrug, so it doesn't need any enzyme to activate it. The concern works the other way around: tricyclics like Clomipramine may slow down how your gut empties, which could reduce how much levodopa from the cowhage actually gets absorbed.

There's also a smaller, theoretical worry from a few old case reports of people on levodopa plus a TCA developing higher blood pressure and extra involuntary movements (dyskinesia). This is theoretical and based on pharmacology, not strong proof, so don't panic. Still, please run it by your pharmacist or doctor before combining them.

Clomipramine — sold as Anafranil and under the generic name Clomipramine Hydrochloride — is approved to treat obsessions and compulsions in people with Obsessive-Compulsive Disorder (OCD). This includes adults and children/adolescents ages 10 to 17.

To qualify for treatment, the obsessions or compulsions must cause significant distress, take up a lot of time, or noticeably interfere with work, school, or social life.
