Midazolam falls under ANESTHESIA — the group Cowhage interacts with.

Theoretically, concomitant use of cowhage and anesthesia might increase the risk of arrhythmias.
Cowhage contains levodopa. Use of levodopa with cyclopropane or halogenated hydrocarbon anesthesia has led to arrhythmias. Other anesthetics have not been implicated. Use other anesthetics in patients taking cowhage or tell patients to stop taking cowhage at least 2 weeks before surgery.

Here's the honest picture: the worry on file is about cowhage's natural levodopa mixing with certain anesthesia gases (like cyclopropane or some halogenated agents), which have been linked to irregular heartbeats. Midazolam is a bit different. It's a benzodiazepine sedative used to calm you and ease anxiety before procedures, not one of those inhaled gases. So the specific arrhythmia concern really points at the gas anesthetics that might be used alongside it, not midazolam itself.

This is theoretical and based on anecdotal reports, so I don't want to alarm you. Still, anytime you have surgery, your care team needs the full list. Please tell your surgeon, anesthesiologist, and pharmacist you take cowhage well ahead of time, since they often have you pause it before procedures.

About Midazolam

What Is Midazolam Used For?

Brand names: Versed, Seizalam, Nayzilam

Midazolam is used in several different forms depending on the setting. Injectable midazolam (given by IV or IM shot) is used before and during surgery or procedures — such as endoscopy, cardiac catheterization, or bronchoscopy — to cause sedation, reduce anxiety, and prevent memory of the event. It can also be used to start general anesthesia or to keep heavily sedated, ventilated patients comfortable in a critical care unit. Midazolam HCl oral syrup is used specifically in children for sedation and anxiety relief before procedures or anesthesia induction, in a monitored medical setting only.

Nayzilam nasal spray is indicated for a different purpose: the acute treatment of seizure clusters (also called acute repetitive seizures) in people aged 12 and older who have epilepsy. It is used intermittently — not as a daily seizure medicine — when a cluster of seizures breaks through the person's usual pattern.
