Nortriptyline falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Sage interacts with.

Theoretically, sage might increase the levels and clinical effects of drugs metabolized by CYP2D6.
In vitro evidence suggests that aqueous extracts of sage can inhibit CYP2D6. So far, this interaction has not been reported in humans.

Here's the gist: your body relies on a liver enzyme called CYP2D6 to break down and clear nortriptyline. In lab dish studies, sage seems to slow that enzyme down. If that happened in real life, nortriptyline could build up a bit more than usual, which might mean stronger effects and more of the side effects this medicine is known for, things like dry mouth, constipation, drowsiness, blurry vision, dizziness on standing, or a faster heartbeat.

The reassuring part: this is only a theoretical concern from test-tube work, and it has never actually been reported in people. Cooking with sage in your food is very unlikely to matter. Concentrated sage extracts or the essential oil are a different story. If you take those, it's worth a quick word with your pharmacist or doctor first.

Here's the idea behind this one. Sage may gently raise levels of a brain and body chemical called acetylcholine. Nortriptyline, on the other hand, has anticholinergic activity, meaning it tends to block that same chemical. So in theory, sage could nudge against some of nortriptyline's effects, and that's where the possible interaction comes from.

I want to be honest about how thin this evidence is. It comes only from lab dish studies, not from people, so we really don't know if it matters in real life. Cooking with sage almost certainly won't cause trouble. Concentrated sage extracts or oils are the bigger question mark. If you're using a sage supplement regularly, just run it by your pharmacist or doctor before mixing the two.

Sage is a common kitchen herb that is generally safe in food amounts and is traditionally used for sore throats, digestion, sweating, and memory. Some early research is encouraging for sore throat and menopausal hot flashes, but most uses are not well proven. Concentrated extracts and the essential oil should be used carefully because high doses contain a compound called thujone, which can be harmful.

Nortriptyline is approved to relieve symptoms of depression. This applies to nortriptyline hydrochloride capsules, nortriptyline hydrochloride oral solution, and Pamelor capsules — all used for the same core indication.

The label notes that depression arising from within the body (endogenous depression) may respond better than other types of depression, though all of these products are indicated for the broader relief of depressive symptoms.
