Indomethacin falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Sage interacts with.

Theoretically, sage might increase the levels and clinical effects of drugs metabolized by CYP2C9.
In vitro evidence suggests that aqueous extracts of sage can inhibit CYP2C9. So far, this interaction has not been reported in humans.

Here's the gist with sage and indomethacin: your liver uses an enzyme called CYP2C9 to help clear this medicine. In lab tests, sage extracts seem to slow that enzyme down. If that happened in the body, indomethacin could hang around a bit longer and build up, which might mean stronger drug effects and a higher chance of side effects like stomach upset, heartburn, ulcers or bleeding, headache, dizziness, or fluid retention.

The important part is how weak this evidence is. It comes only from test-tube studies, and it has never actually been reported in people. Sprinkling sage on your dinner is very unlikely to matter. Concentrated sage extracts or capsules are the bigger question, so check with your pharmacist or doctor before starting them.

Sage is a common kitchen herb that is generally safe in food amounts and is traditionally used for sore throats, digestion, sweating, and memory. Some early research is encouraging for sore throat and menopausal hot flashes, but most uses are not well proven. Concentrated extracts and the essential oil should be used carefully because high doses contain a compound called thujone, which can be harmful.

About Indomethacin

What Is Indomethacin Used For?

Brand names: Indocin, Indocin SR, Indo-Lemmon, Indomethagan, Tivorbex

Indomethacin oral suspension and rectal suppository forms (including Indocin oral suspension and Indocin suppositories) are used to treat moderate to severe rheumatoid arthritis (including acute flares), moderate to severe ankylosing spondylitis (spine inflammation), moderate to severe osteoarthritis, acute painful shoulder (bursitis and/or tendinitis), and acute gouty arthritis. Oral capsules — both immediate-release and extended-release — share these same approved uses, except that extended-release capsules are not approved for acute gouty arthritis.

Indomethacin for Injection (intravenous) is a completely separate use: it is given to premature infants to close a hemodynamically significant patent ductus arteriosus (a blood vessel near the heart that should close after birth but hasn't) when standard medical management has not worked.
