Phenytoin falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Ipriflavone interacts with.

Ipriflavone is thought to competitively inhibit cytochrome P450 2C9 (CYP2C9). Theoretically, concurrent use of ipriflavone and drugs metabolized by these enzymes might result in decreased drug elimination, increased serum levels, and potential toxicity. Some drugs metabolized by CYP2C9 include amitriptyline (Elavil), diazepam (Valium), estradiol (Estrace), tacrine (Cognex), verapamil (Calan), warfarin (Coumadin), zileuton (Zyflo), and others.

Here's the gist for phenytoin (Dilantin). Your body relies on a liver enzyme called CYP2C9 to clear this seizure medicine out. Ipriflavone is thought to slow that same enzyme down, so in theory phenytoin could build up in your blood instead of leaving at its normal pace. That matters more than usual because phenytoin is a narrow therapeutic index drug, meaning the gap between a helpful level and a too-high level is small.

If levels climbed, you might notice dizziness, unsteady walking, slurred speech, blurred or jumping vision, or feeling unusually drowsy. I'll be honest, though: this concern comes from lab studies only, not real patients, so it may not play out in practice. Don't change anything on your own. Just check with your pharmacist or doctor, who can monitor your phenytoin level if needed.

About Phenytoin

What Is Phenytoin Used For?

Brand names: Dilantin Kapseals, Dilantin, Di-Phen, Dilantin Infatabs, Phenytek, Dilantin injection

Phenytoin is used to treat and prevent seizures. The specific types covered depend on the formulation. Oral extended-release capsules (including Phenytek), chewable tablets (including Dilantin Infatabs and Phenytoin Infatabs), and oral suspension (including Dilantin-125) are used for tonic-clonic (grand mal) and psychomotor (temporal lobe) seizures; most oral forms also cover prevention and treatment of seizures occurring during or after neurosurgery. Chewable tablets and some capsule formulations also cover complex partial seizures.

Parenteral (injectable) phenytoin sodium is used for generalized tonic-clonic status epilepticus (a prolonged, life-threatening seizure emergency) and for prevention and treatment of seizures during neurosurgery. The IV form can also substitute short-term for oral phenytoin when a patient cannot take it by mouth.
