Piroxicam falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Ipriflavone interacts with.

Ipriflavone is thought to competitively inhibit cytochrome P450 2C9 (CYP2C9). Theoretically, concurrent use of ipriflavone and drugs metabolized by these enzymes might result in decreased drug elimination, increased serum levels, and potential toxicity. Some drugs metabolized by CYP2C9 include amitriptyline (Elavil), diazepam (Valium), estradiol (Estrace), tacrine (Cognex), verapamil (Calan), warfarin (Coumadin), zileuton (Zyflo), and others.

Here's the gist for piroxicam (Feldene): your liver relies on an enzyme called CYP2C9 to clear this medicine out of your system. Ipriflavone is thought to slow that enzyme down, so in theory piroxicam could hang around longer and build up to higher levels than usual.

If that happens, you might notice more of piroxicam's typical side effects, things like stomach upset, heartburn, a higher chance of stomach irritation or bleeding, fluid retention, or kidney strain. I want to be honest, though: this concern comes from lab and test-tube studies, not from real patients, so it's more of a theoretical caution than a proven problem. Still, since piroxicam is already harder on the stomach than many NSAIDs, it's worth running this combination past your pharmacist or doctor before pairing them.
