Why this applies to Lonafarnib

Lonafarnib falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Limonene interacts with.

There's preliminary evidence that limonene might be a substrate for cytochrome P450 2C9 (CYP2C9), as well causing its induction. So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP2C9 in patients taking limonene. Some drugs metabolized by CYP2C9 include nonsteroidal anti-inflammatory drugs (NSAIDs) such as diclofenac (Cataflam, Voltaren), ibuprofen (Motrin), meloxicam (Mobic), and piroxicam (Feldene); celecoxib (Celebrex); amitriptyline (Elavil); warfarin (Coumadin); glipizide (Glucotrol); losartan (Cozaar); and others.

Here's the gist for Zokinvy (lonafarnib), the medicine used to slow down the progeria aging disorder. Lab and animal research hints that limonene, the oil from citrus peels, might speed up an enzyme called CYP2C9. If a supplement nudges that enzyme to work faster, drugs it handles can get cleared out a little quicker, meaning lower levels in your blood.

So in theory, concentrated limonene could make lonafarnib a touch less effective, which matters for a drug you're counting on to do its job. I want to be honest though: this has not been seen in real people, and the evidence is only test-tube and animal level. The citrus you eat in food almost certainly won't matter. Before adding a limonene supplement, please run it by your doctor or pharmacist first.

What Is Lonafarnib Used For?

Brand name: Zokinvy

Zokinvy (lonafarnib) is approved for patients 12 months of age and older who have a body surface area of 0.39 m² or above. It is used to reduce the risk of mortality in Hutchinson-Gilford Progeria Syndrome (HGPS) — a rare, severe premature-aging disorder.

Zokinvy is also used to treat processing-deficient Progeroid Laminopathies — a related group of rare genetic conditions — specifically those caused by a heterozygous LMNA mutation with progerin-like protein buildup, or by homozygous or compound heterozygous ZMPSTE24 mutations. It is not indicated for other Progeroid Syndromes or processing-proficient Progeroid Laminopathies.
