Indomethacin falls under CYTOCHROME P450 2C19 (CYP2C19) INHIBITORS — the group Limonene interacts with.

There's preliminary evidence that limonene might be a substrate for cytochrome P450 2C19 (CYP2C19). So far, this interaction has not been reported in humans. However, watch for an increase in the limonene levels when it is taken with drugs that inhibit CYP2C19. Some drugs that inhibit CYP2C19 include cimetidine (Tagamet), fluvoxamine (Luvox), omeprazole (Prilosec); ticlopidine (Ticlid), topiramate (Topamax), and others.

Here's the key thing to understand about this pairing: the concern flows toward the limonene, not your indomethacin. Indomethacin can act as a mild blocker of an enzyme called CYP2C19, and early lab work suggests limonene may be broken down by that same enzyme. So in theory, taking them together could let limonene levels creep up a bit. Your indomethacin itself is not a prodrug and is not made stronger or weaker by this, so your arthritis or gout relief shouldn't change.

I'll be honest with you: this is based only on test-tube and animal data, and it has never actually been reported in people, so I wouldn't lose sleep over it. Food amounts of citrus are no worry. If you're using concentrated limonene capsules, just run it past your pharmacist.

Indomethacin also falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES.

There's preliminary evidence that limonene might be a substrate for cytochrome P450 2C9 (CYP2C9), as well causing its induction. So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP2C9 in patients taking limonene. Some drugs metabolized by CYP2C9 include nonsteroidal anti-inflammatory drugs (NSAIDs) such as diclofenac (Cataflam, Voltaren), ibuprofen (Motrin), meloxicam (Mobic), and piroxicam (Feldene); celecoxib (Celebrex); amitriptyline (Elavil); warfarin (Coumadin); glipizide (Glucotrol); losartan (Cozaar); and others.

Here's the gist for indomethacin (Indocin): your body relies on a liver enzyme called CYP2C9 to help break it down. Early lab work suggests limonene might speed that enzyme up a bit. If that happened, your body could clear indomethacin a little faster, which in theory might make it work less well, so you could notice more joint pain, stiffness, or swelling sneaking back in.

I want to be honest about the strength here, though. This comes only from test-tube and animal studies, and it has never been reported in people. The small amount of limonene in an orange or a glass of lemonade is very unlikely to matter. If you're using a concentrated limonene supplement, just run it by your pharmacist or doctor first.

About Indomethacin

What Is Indomethacin Used For?

Brand names: Indocin, Indocin SR, Indo-Lemmon, Indomethagan, Tivorbex

Indomethacin oral suspension and rectal suppository forms (including Indocin oral suspension and Indocin suppositories) are used to treat moderate to severe rheumatoid arthritis (including acute flares), moderate to severe ankylosing spondylitis (spine inflammation), moderate to severe osteoarthritis, acute painful shoulder (bursitis and/or tendinitis), and acute gouty arthritis. Oral capsules — both immediate-release and extended-release — share these same approved uses, except that extended-release capsules are not approved for acute gouty arthritis.

Indomethacin for Injection (intravenous) is a completely separate use: it is given to premature infants to close a hemodynamically significant patent ductus arteriosus (a blood vessel near the heart that should close after birth but hasn't) when standard medical management has not worked.
