Tapentadol falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Limonene interacts with.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the honest picture with limonene and tapentadol. The concern on file is that limonene might speed up an enzyme called CYP2C9, which could lower the levels of drugs that depend on it. The catch is that tapentadol leans mostly on a different pathway (a process called glucuronidation), with only minor involvement from enzymes like CYP2C9, so this particular interaction is unlikely to amount to much.
On top of that, the evidence here comes only from lab and animal studies, not from people, so it's quite theoretical. In the worst case you might notice slightly weaker pain control, but that's far from certain. Food amounts of citrus are not a worry. If you're using concentrated limonene supplements, just run it by your pharmacist or doctor first.

What Is Tapentadol Used For?
Brand names: Nucynta, Nucynta ER
Tapentadol is an opioid pain reliever. It is only for pain that is severe enough to need an opioid and that other treatments can't manage well.
- Tablets and oral solution: acute (short-term) severe pain in adults. Nucynta tablets and tapentadol tablets are also labeled for children aged 6 and older who weigh at least 40 kg. The oral solution labeling does not carry that pediatric use.
- Extended-release tablets (Nucynta ER and tapentadol extended-release): severe, ongoing pain in adults that needs daily, long-term opioid treatment. They are also used for severe nerve pain from diabetic peripheral neuropathy (nerve damage from diabetes) in adults. They are not for as-needed use.
