Limonene + Alpelisib
Alpelisib brand name: Piqray.
Limonene has a moderate interaction with Alpelisib. There's preliminary evidence that limonene might be a substrate for cytochrome P450 2C9 (CYP2C9).

Here is the interaction we found between these two, with the evidence behind it.
CYTOCHROME P450 2C9 (CYP2C9) INDUCERS
Why this applies to Alpelisib
Alpelisib falls under CYTOCHROME P450 2C9 (CYP2C9) INDUCERS — the group Limonene interacts with.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the important thing to understand about this one: the interaction on file describes how limonene itself gets broken down, not how it affects your alpelisib. Limonene appears to be processed by a liver enzyme called CYP2C9, and drugs that speed up that enzyme could lower limonene levels. This is really about the supplement's fate, not your cancer medicine.
So far there's no solid evidence that limonene meaningfully changes how alpelisib (Piqray) works in your body. The data behind this entry comes only from lab and animal studies, so it's preliminary. Still, alpelisib is a serious cancer drug with real side effects like high blood sugar, rash, and diarrhea, so I'd always run any new supplement past your oncologist or pharmacist first before combining the two.

- ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
- PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
- UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.

From our pharmacist’s overview on the Alpelisib page
“Alpelisib (Piqray) treats PIK3CA-mutated breast cancer, and a drug doing that job leaves little room for supplements that shift its blood level. Yet that is what most of this list does: 121 of the 139 ingredients on file can raise or lower alpelisib through CYP3A4, the liver enzyme that clears it.”

- Heartburn and acid reflux
- Digestive upset
- Stress and mood support
- Antioxidant support
- General wellness
Limonene is a natural compound found in the peel of citrus fruits like oranges and lemons, and it is used as a flavoring agent and dietary supplement. About Alpelisib
What Is Alpelisib Used For?
Brand name: Piqray
Alpelisib is approved for two distinct uses, each tied to a specific brand. Piqray is used in combination with another drug called fulvestrant to treat adults with hormone receptor (HR)-positive, HER2-negative, advanced or metastatic breast cancer whose tumors carry a PIK3CA mutation — and only after the cancer has progressed on or after a hormone-based treatment. A companion diagnostic test is required before starting Piqray.
Vijoice is used to treat adults and children 2 years of age and older who have severe manifestations of PIK3CA-Related Overgrowth Spectrum (PROS) — a rare condition where PIK3CA gene mutations cause abnormal tissue, vascular, and bone overgrowth — and who need systemic (whole-body) therapy. Vijoice's approval for PROS was granted under the FDA's accelerated approval pathway based on response data, and continued approval may depend on confirmatory trials.
