Phenytoin falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Limonene interacts with.

There's preliminary evidence that limonene might be a substrate for cytochrome P450 2C9 (CYP2C9), as well causing its induction. So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP2C9 in patients taking limonene. Some drugs metabolized by CYP2C9 include nonsteroidal anti-inflammatory drugs (NSAIDs) such as diclofenac (Cataflam, Voltaren), ibuprofen (Motrin), meloxicam (Mobic), and piroxicam (Feldene); celecoxib (Celebrex); amitriptyline (Elavil); warfarin (Coumadin); glipizide (Glucotrol); losartan (Cozaar); and others.

Here's the gist for phenytoin (Dilantin). Your body clears it partly through a liver enzyme called CYP2C9. Early lab work hints that limonene might speed that enzyme up (induce it). If that happened, your phenytoin could be cleared a little faster, which in theory might nudge your blood levels down and make your seizures harder to control.

Now, the honest part: this is only test-tube and animal data, and it has never actually been reported in people. So I wouldn't panic. But phenytoin is a tricky drug where even small swings matter, so don't start a concentrated limonene supplement without telling your pharmacist or doctor first. A squeeze of citrus in your food is not a concern.

There's preliminary evidence that limonene might be a substrate for cytochrome P450 2C19 (CYP2C19). CYP2C19 inducers might decrease the effects of limonene. So far, this interaction has not been reported in humans. Inducers of CYP2C19 include carbamazepine (Tegretol), prednisone (Deltasone), and rifampin (Rifadin, Rimactane).

Here's the key thing to understand about this pairing: the concern flows from phenytoin onto limonene, not the other way around. Phenytoin is one of the medicines that can speed up a liver enzyme called CYP2C19. Early lab work hints that limonene may be broken down by that same enzyme, so a CYP2C19 booster like phenytoin could clear limonene out faster and make the supplement less effective. There's no good reason to expect limonene to change your phenytoin levels here.

That said, this is only test-tube and animal data, and it hasn't shown up in real people, so I wouldn't lose sleep over it. Citrus-flavored foods are not a worry. Here's the key thing to understand about this pairing: the interaction runs in the direction of limonene, not your phenytoin. Phenytoin is a known booster (an inducer) of the liver enzyme CYP2C9, and lab and animal work hints that limonene is broken down by that same enzyme. So in theory, taking phenytoin could speed up how fast your body clears limonene, leaving you with less of the supplement than you'd expect. That's the whole story here.

The reassuring part is your phenytoin levels are not expected to change, so this shouldn't affect your seizure control or cause phenytoin side effects like dizziness, unsteadiness, or drowsiness. This is also very early, test-tube level evidence, and food amounts of citrus are not a concern. Still, since phenytoin is a finicky drug with a narrow safe range, run any concentrated limonene supplement by your pharmacist or doctor first.

About Phenytoin

What Is Phenytoin Used For?

Brand names: Dilantin Kapseals, Dilantin, Di-Phen, Dilantin Infatabs, Phenytek, Dilantin injection

Phenytoin is used to treat and prevent seizures. The specific types covered depend on the formulation. Oral extended-release capsules (including Phenytek), chewable tablets (including Dilantin Infatabs and Phenytoin Infatabs), and oral suspension (including Dilantin-125) are used for tonic-clonic (grand mal) and psychomotor (temporal lobe) seizures; most oral forms also cover prevention and treatment of seizures occurring during or after neurosurgery. Chewable tablets and some capsule formulations also cover complex partial seizures.

Parenteral (injectable) phenytoin sodium is used for generalized tonic-clonic status epilepticus (a prolonged, life-threatening seizure emergency) and for prevention and treatment of seizures during neurosurgery. The IV form can also substitute short-term for oral phenytoin when a patient cannot take it by mouth.
