Why this applies to Dronabinol

Dronabinol falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Limonene interacts with.

There's preliminary evidence that limonene might be a substrate for cytochrome P450 2C9 (CYP2C9), as well causing its induction. So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP2C9 in patients taking limonene. Some drugs metabolized by CYP2C9 include nonsteroidal anti-inflammatory drugs (NSAIDs) such as diclofenac (Cataflam, Voltaren), ibuprofen (Motrin), meloxicam (Mobic), and piroxicam (Feldene); celecoxib (Celebrex); amitriptyline (Elavil); warfarin (Coumadin); glipizide (Glucotrol); losartan (Cozaar); and others.

Here's the gist for Dronabinol (Marinol, Syndros): your body uses a liver enzyme called CYP2C9 to help clear it. There's early lab and animal evidence that limonene might rev up (induce) that enzyme. If that happened in people, it could speed up how fast dronabinol is broken down, which may leave you with less of it working. In practice that could mean your nausea, vomiting, or poor appetite isn't controlled as well as you'd expect.

The important caveat: this has only been seen in test tubes and animals, never confirmed in humans, so I wouldn't lose sleep over it. Food amounts of citrus are very unlikely to matter. If you're taking a concentrated limonene supplement, just run it by your pharmacist or doctor first.

What Is Dronabinol Used For?

Brand names: Marinol, Syndros

Dronabinol — available as capsules (Marinol, Dronabinol) and as an oral solution (Syndros, Dronabinol oral solution) — is approved for two conditions in adults. First, it treats anorexia (severely reduced appetite) associated with weight loss in patients with AIDS. Second, it controls nausea and vomiting caused by cancer chemotherapy in patients who have not gotten enough relief from standard anti-nausea treatments.

Both uses apply to all available formulations, but dosing timing and meal instructions differ between the capsule and oral solution products. Always follow the specific instructions for the product you have been prescribed.
