Here's the gist for dronabinol (Marinol, Syndros): your liver clears it using an enzyme called CYP3A4. St. John's wort revs that enzyme up, so your body breaks dronabinol down faster than usual. The likely result is lower drug levels, which means it may not work as well for the nausea, vomiting, or poor appetite you're trying to treat.

This matters because dronabinol isn't a prodrug that needs activating, so faster clearance simply means weaker effect, not a buildup of side effects. The concern here is treatment failure. This is a well-recognized interaction with concentrated supplement doses (it's the hyperforin in St. John's wort that drives it). Please don't stop or change anything on your own. Talk with your pharmacist or doctor first so they can guide you.

St. John's wort may increase the metabolism and reduce the levels of CYP2C9 substrates.
There is contradictory research about the effect of St. John's wort on CYP2C9. Some in vitro research shows that St. John's wort induces CYP2C9, but to a lesser extent than CYP3A4. St. John's wort also induces metabolism of the S-warfarin isomer, which is a CYP2C9 substrate. Other research shows that St. John's wort 300 mg three times daily for 21 days does not significantly affect the pharmacokinetics of a single 400 mg dose of ibuprofen, which is also a CYP2C9 substrate. Until more is known, use St. John's wort cautiously in patients who are taking CYP2C9 substrates.

Here's the gist for dronabinol (Marinol, Syndros). Your liver uses an enzyme called CYP2C9 to help break this medicine down. St. John's wort tends to speed that enzyme up, so it could clear dronabinol out of your system a bit faster. In plain terms, your dronabinol might not work as well, meaning less relief from nausea and vomiting, or less help with appetite and weight, which really matters if you're going through chemo or living with AIDS.

I'll be straight with you: the evidence here is mixed and not super strong, so this may or may not be a big deal for any one person. Still, since dronabinol is doing important work, please loop in your pharmacist or doctor before mixing the two rather than changing anything on your own.

What Is Dronabinol Used For?

Brand names: Marinol, Syndros

Dronabinol — available as capsules (Marinol, Dronabinol) and as an oral solution (Syndros, Dronabinol oral solution) — is approved for two conditions in adults. First, it treats anorexia (severely reduced appetite) associated with weight loss in patients with AIDS. Second, it controls nausea and vomiting caused by cancer chemotherapy in patients who have not gotten enough relief from standard anti-nausea treatments.

Both uses apply to all available formulations, but dosing timing and meal instructions differ between the capsule and oral solution products. Always follow the specific instructions for the product you have been prescribed.
