Dronabinol falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Adrafinil interacts with.

Theoretically, adrafinil might increase the levels and clinical effects of CYP2C9 substrates.
Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil has been shown to reduce CYP2C9 expression in vitro.

Here's the gist for these two together. Your liver uses an enzyme called CYP2C9 to help break down dronabinol. Adrafinil turns into modafinil in your body, and lab studies hint that modafinil may slow this enzyme down a bit. If that happened, dronabinol could clear more slowly and build up, which can mean stronger effects than you expect.

For dronabinol that might show up as more drowsiness, dizziness, a fuzzy or 'high' feeling, a racing heart, or feeling lightheaded when you stand. I want to be honest though: this is only a theoretical concern from test-tube data, not something proven in people, so it may never cause a real problem. Still, since adrafinil is unregulated and hard on the liver, please run this combo by your pharmacist or doctor first.

Theoretically, adrafanil might reduce levels and clinical effects of CYP3A4 substrates.

Here's the gist for these two together. Your body turns adrafinil into modafinil, and modafinil can gently speed up an enzyme in your liver called CYP3A4. That same enzyme is one of the main ways your body breaks down dronabinol (Marinol, Syndros). So in theory, taking adrafinil could clear dronabinol out a little faster, leaving you with lower levels.

What that might look like in real life is dronabinol simply not working as well, so more chemo-related nausea, vomiting, or poor appetite could slip through. I'll be honest though: this is a theoretical concern based on how the drugs behave, not something proven in people, and modafinil is only a weak inducer. Still, please loop in your doctor or pharmacist before combining them so they can keep an eye on how well your dronabinol is controlling your symptoms.

What Is Dronabinol Used For?

Brand names: Marinol, Syndros

Dronabinol — available as capsules (Marinol, Dronabinol) and as an oral solution (Syndros, Dronabinol oral solution) — is approved for two conditions in adults. First, it treats anorexia (severely reduced appetite) associated with weight loss in patients with AIDS. Second, it controls nausea and vomiting caused by cancer chemotherapy in patients who have not gotten enough relief from standard anti-nausea treatments.

Both uses apply to all available formulations, but dosing timing and meal instructions differ between the capsule and oral solution products. Always follow the specific instructions for the product you have been prescribed.
