SEROTONERGIC DRUGS
Why this applies to Tapentadol
Tapentadol falls under SEROTONERGIC DRUGS — the group Lithium interacts with.
In a case report, a 67-year-old female with depression and bipolar disorder using lithium in combination with selective serotonin reuptake inhibitors (SSRIs) and other medications developed serotonin syndrome with symptoms of deep tendon hyperreflexia, muscle rigidity, tremor, and hyperthermia. However, agitation, one classical symptom of serotonin syndrome, was lacking. This was thought to be due to masking by lithium toxicity. Lithium can increase serotonin levels, thus, combining serotonergic drugs with lithium might increase the risk of serotonergic side effects including serotonin syndrome and cerebral vasoconstrictive disorders. It is unclear if this interaction would occur with the smaller doses found in lithium supplements.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the thing about combining lithium with tapentadol: tapentadol doesn't just block opioid receptors, it also raises norepinephrine and nudges serotonin signaling in your nervous system. Lithium can push serotonin levels up too. Stack the two together and, in theory, you raise the odds of serotonin syndrome, which can show up as agitation, shaking or tremor, muscle stiffness, a racing heart, sweating, or a high body temperature.
I want to be honest about the evidence here. This warning is mostly theoretical and based on how these drugs behave, plus one case report involving prescription-strength lithium. We don't really know if the small amounts in lithium orotate supplements matter much. Still, because serotonin syndrome is serious, please run this combination by your pharmacist or doctor before mixing them.

Lithium is a naturally occurring metal that, in prescription form (lithium carbonate or citrate), is an FDA-approved, well-proven treatment for bipolar disorder. Low-dose supplement forms like lithium orotate are sold for mood support, but there is very little reliable evidence that they work, and even small amounts of lithium can be risky for some people. About Tapentadol
What Is Tapentadol Used For?
Brand names: Nucynta, Nucynta ER
Tapentadol is an opioid pain reliever. It is only for pain that is severe enough to need an opioid and that other treatments can't manage well.
- Tablets and oral solution: acute (short-term) severe pain in adults. Nucynta tablets and tapentadol tablets are also labeled for children aged 6 and older who weigh at least 40 kg. The oral solution labeling does not carry that pediatric use.
- Extended-release tablets (Nucynta ER and tapentadol extended-release): severe, ongoing pain in adults that needs daily, long-term opioid treatment. They are also used for severe nerve pain from diabetic peripheral neuropathy (nerve damage from diabetes) in adults. They are not for as-needed use.
