Why this applies to Phenytoin
Phenytoin falls under ANTICONVULSANTS — the group Lithium interacts with.
Drugs such as carbamazepine and phenytoin seem to increase the risk of neurotoxicity. It is unclear if this interaction would occur with the smaller doses found in lithium supplements.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the situation with phenytoin (Dilantin) and lithium. Both of these can affect the nervous system, and on paper, combining them might raise the chance of what we call neurotoxicity. In real terms, that could look like worsening dizziness, unsteady walking, slurred speech, confusion, tremor, or that drunk-feeling clumsiness. Phenytoin already has a narrow safe range, so it's a drug we watch carefully.
I want to be honest about the evidence here: this is theoretical, based on how the drugs work rather than strong studies. Most of the concern comes from prescription-strength lithium, not the tiny amounts in supplements like lithium orotate. Still, since both touch the brain, please run any lithium product by your pharmacist or doctor first.

Lithium is a naturally occurring metal that, in prescription form (lithium carbonate or citrate), is an FDA-approved, well-proven treatment for bipolar disorder. Low-dose supplement forms like lithium orotate are sold for mood support, but there is very little reliable evidence that they work, and even small amounts of lithium can be risky for some people. What Is Phenytoin Used For?
Brand names: Dilantin Kapseals, Dilantin, Di-Phen, Dilantin Infatabs, Phenytek, Dilantin injection
Phenytoin treats seizures. Oral products are used for generalized tonic-clonic (grand mal) seizures and for psychomotor (temporal lobe) or complex partial seizures. Most oral products are also used to prevent and treat seizures during or after neurosurgery.
Phenytoin Sodium Injection is used for generalized tonic-clonic status epilepticus (a prolonged, dangerous seizure emergency). It is also used for seizures during neurosurgery. The injection should be used only when taking phenytoin by mouth is not possible. Dilantin-125 and Phenytoin oral suspension are listed for tonic-clonic and psychomotor seizures.
