Venlafaxine falls under SEROTONERGIC DRUGS — the group Lithium interacts with.

Theoretically, taking lithium supplements with serotonergic drugs might both mask and increase the risk of serotonin syndrome.
In a case report, a 67-year-old female with depression and bipolar disorder using lithium in combination with selective serotonin reuptake inhibitors (SSRIs) and other medications developed serotonin syndrome with symptoms of deep tendon hyperreflexia, muscle rigidity, tremor, and hyperthermia. However, agitation, one classical symptom of serotonin syndrome, was lacking. This was thought to be due to masking by lithium toxicity. Lithium can increase serotonin levels, thus, combining serotonergic drugs with lithium might increase the risk of serotonergic side effects including serotonin syndrome and cerebral vasoconstrictive disorders. It is unclear if this interaction would occur with the smaller doses found in lithium supplements.

Here's the concern with combining a lithium supplement and venlafaxine (Effexor). Venlafaxine already boosts serotonin in your brain, and lithium can nudge serotonin levels up too. Stack the two and you raise the odds of too much serotonin activity, a problem we call serotonin syndrome. Warning signs include shakiness or tremor, muscle stiffness, racing heart, sweating, fever, restlessness, or confusion.

One tricky detail from a case report: lithium can actually mask some of these symptoms, so trouble may be harder to spot. Keep in mind this warning is mostly theoretical and based on pharmacology, and it's unclear whether the low doses in supplements like lithium orotate matter. Still, please check with your pharmacist or doctor before pairing them, and don't change your venlafaxine on your own.

Venlafaxine is used to treat major depressive disorder (MDD) across all available formulations. Extended-release capsules (including Effexor XR) are also approved for generalized anxiety disorder (GAD), social anxiety disorder (SAD), and panic disorder (PD).

Extended-release tablets are approved for major depressive disorder and social anxiety disorder. Immediate-release venlafaxine tablets are indicated for major depressive disorder only.
