Theophylline falls under METHYLXANTHINES — the group Lithium interacts with.

Theoretically, taking lithium supplements with methylxanthines might decrease lithium levels.
Xanthines such as aminophylline, caffeine, and theophylline (Theo-Dur, Theo-24, others) might increase the clearance of lithium. It is unclear if this interaction would be clinically significant with the smaller doses found in lithium supplements.

Here's the interesting twist with this pair: it's the lithium that may take the hit, not your theophylline. Theophylline (and related drugs like caffeine and aminophylline) can speed up how fast your kidneys clear lithium out of your body. So if you're taking a lithium product for mood support, theophylline could nudge your lithium levels down a bit and make it work less well.

Now, the honest part: this is theoretical, based on how these drugs behave rather than strong studies. With the small amounts in lithium orotate supplements, it may not matter much at all. Theophylline isn't a prodrug here, so this isn't about changing your breathing medicine. Still, lithium is tricky stuff, so please check with your pharmacist or doctor before combining them.

Theophylline — available as extended-release capsules (Theo-24), extended-release tablets, and oral solution (Elixophyllin) — is used to treat symptoms and reversible airflow obstruction caused by chronic asthma, emphysema, and chronic bronchitis.

All supported formulations share these indications. Theophylline is intended for patients with ongoing or recurring symptoms, not for sudden (acute) breathing attacks, which need faster-acting treatments.
