Why this applies to Linezolid

Linezolid falls under SEROTONERGIC DRUGS — the group Lithium interacts with.

Theoretically, taking lithium supplements with serotonergic drugs might both mask and increase the risk of serotonin syndrome.
In a case report, a 67-year-old female with depression and bipolar disorder using lithium in combination with selective serotonin reuptake inhibitors (SSRIs) and other medications developed serotonin syndrome with symptoms of deep tendon hyperreflexia, muscle rigidity, tremor, and hyperthermia. However, agitation, one classical symptom of serotonin syndrome, was lacking. This was thought to be due to masking by lithium toxicity. Lithium can increase serotonin levels, thus, combining serotonergic drugs with lithium might increase the risk of serotonergic side effects including serotonin syndrome and cerebral vasoconstrictive disorders. It is unclear if this interaction would occur with the smaller doses found in lithium supplements.

Here's something a lot of people don't realize about linezolid: besides being an antibiotic, it also acts like a mild MAO inhibitor, which means it can raise serotonin levels in your body. Lithium can nudge serotonin upward too. Put the two together and you could, in theory, raise the risk of serotonin syndrome, which shows up as things like agitation, muscle twitching or stiffness, tremor, sweating, racing heart, and fever.

One extra wrinkle: lithium may actually mask some of the early warning signs, so trouble could sneak up on you. That said, this concern is mostly theoretical, and we don't know if the small amounts in lithium supplements matter. Still, please check with your pharmacist or doctor before combining them.

About Linezolid

What Is Linezolid Used For?

Brand names: Zyvox, Zyvox Injection

Linezolid — available as Zyvox and in generic forms — is used to treat infections caused by certain drug-resistant Gram-positive bacteria. It is approved for adults and children for nosocomial pneumonia (hospital-acquired lung infection), community-acquired pneumonia, complicated skin and skin structure infections (including diabetic foot infections, but only when bone infection is not also present), uncomplicated skin and skin structure infections, and vancomycin-resistant Enterococcus faecium infections.

It is important to know that linezolid does not work against Gram-negative bacteria. If a Gram-negative infection is also present or suspected, a separate antibiotic must be started right away. Linezolid should only be used for infections that are confirmed or strongly suspected to be caused by bacteria, to help prevent antibiotic resistance.
