Why this applies to Tapentadol

Tapentadol falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Quercetin interacts with.

Theoretically, concomitant use might increase the levels and adverse effects of CYP2C9 substrates.

A small clinical study in healthy volunteers shows that taking quercetin 500 mg twice daily for 10 days prior to taking diclofenac, a CYP2C9 substrate, increases diclofenac plasma levels by 75% and prolongs the half-life by 32.5%. Animal research also shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of losartan (Cozaar), a substrate of CYP2C9. Furthermore, laboratory research shows that quercetin inhibits CYP2C9.

Here's the gist: quercetin in supplement doses may gently slow down one of your liver's processing enzymes (CYP2C9). Since your body uses that pathway to help clear tapentadol, the worry is that the drug could hang around a little longer and build up. Tapentadol isn't a prodrug, so more drug in your system means stronger opioid effects, things like extra drowsiness, dizziness, nausea, constipation, or slowed breathing.

Honestly, the evidence here is on the thin side and mostly theoretical, drawn from a small study using a different drug. Food amounts of quercetin (fruits, onions, tea) almost certainly won't matter. Before starting high-dose quercetin capsules alongside tapentadol, run it by your pharmacist or doctor first.

About Tapentadol

What Is Tapentadol Used For?

Brand names: Nucynta, Nucynta ER

Tapentadol tablets (including Nucynta and Tapentadol Hydrochloride tablets) are used for severe acute pain in adults when other treatments are not adequate. Nucynta tablets are also approved for acute pain in children aged 6 and older who weigh at least 40 kg.

Nucynta ER and tapentadol extended-release tablets are used for severe, ongoing pain that requires around-the-clock opioid treatment and cannot be managed with other options, including severe neuropathic pain (nerve pain) caused by diabetic peripheral neuropathy in adults. Extended-release formulations are not intended for as-needed use.
