Tapentadol falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Eucalyptus interacts with.

Theoretically, eucalyptus might increase the levels of CYP2C19 substrates.
In vitro research suggests that eucalyptus oil might inhibit CYP2C19, although this has not been reported in humans.

Here's the gist for tapentadol (Nucynta). Lab studies hint that eucalyptol, the active part of eucalyptus oil, might block a liver enzyme called CYP2C19. In theory, slowing that enzyme could let a CYP2C19-cleared medicine build up a bit. For an opioid like tapentadol, more drug in your system could mean stronger drowsiness, dizziness, constipation, or slowed breathing.

The good news: this is only a test-tube finding, and it has never actually been shown in people. Tapentadol is also handled mostly by other pathways, not chiefly CYP2C19, so a real-world problem is unlikely, especially from a vapor rub or lozenge. Still, never swallow eucalyptus oil, and check with your pharmacist or doctor before combining them.

Here's the gist with eucalyptus and your Tapentadol (Nucynta). In a test tube, eucalyptus oil seems to slow down an enzyme called CYP2C9, which is one of the pathways your body uses to clear this pain medicine. If that pathway slows, Tapentadol could theoretically build up a bit, nudging up its effects and side effects like drowsiness, dizziness, nausea, constipation, or slowed breathing.

I want to be honest about how shaky this is. Tapentadol is an active drug (not a prodrug), so the direction makes sense, but this has only been seen in lab studies and never shown in real people. A whiff of vapor rub or a lozenge almost certainly won't matter. Just loop in your pharmacist or doctor before swallowing any concentrated eucalyptus product.

Eucalyptus is best known for its strong-smelling oil used in vapor rubs, inhalants, and lozenges to ease coughs and congestion. It may provide mild, short-term relief of cold symptoms, but the oil is toxic if swallowed and must never be taken by mouth undiluted. What Is Tapentadol Used For?

Brand names: Nucynta, Nucynta ER

Tapentadol tablets (including Nucynta and Tapentadol Hydrochloride tablets) are used for severe acute pain in adults when other treatments are not adequate. Nucynta tablets are also approved for acute pain in children aged 6 and older who weigh at least 40 kg.

Nucynta ER and tapentadol extended-release tablets are used for severe, ongoing pain that requires around-the-clock opioid treatment and cannot be managed with other options, including severe neuropathic pain (nerve pain) caused by diabetic peripheral neuropathy in adults. Extended-release formulations are not intended for as-needed use.
