Lonafarnib falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Evening Primrose interacts with.

Theoretically, evening primrose may increase the levels and clinical effects of CYP2C9 substrates.
In vitro research shows that linoleic acid, a constituent of evening primrose oil, inhibits CYP2C9.

Here's the gist for Zokinvy (lonafarnib). This is a real medication, given to slow the progression of a rare genetic aging disorder called progeria. The thinking behind this interaction is that a component of evening primrose oil, linoleic acid, may slow down a liver enzyme (CYP2C9) that helps clear certain drugs. If that enzyme slows, lonafarnib could in theory build up a bit, which might mean more of its typical side effects like nausea, vomiting, diarrhea, tiredness, or reduced appetite.

I'll be honest though, this is a theoretical, lab-based concern, not something proven in people, and it's rated minor. Lonafarnib is also a serious medicine handled closely by a specialist, so don't add evening primrose without checking with that team first.

Evening primrose oil is a seed oil rich in gamma-linolenic acid (GLA), an omega-6 fatty acid, that is popularly used for skin conditions, PMS, and breast pain. The evidence behind most of these uses is mixed or weak, and it is not proven to be a reliable treatment for any condition. It is generally well tolerated for short-term use, but talk to your pharmacist or doctor before starting it, especially if you are pregnant or have a bleeding disorder.

What Is Lonafarnib Used For?

Brand name: Zokinvy

Zokinvy (lonafarnib) is approved for patients 12 months of age and older who have a body surface area of 0.39 m² or above. It is used to reduce the risk of mortality in Hutchinson-Gilford Progeria Syndrome (HGPS) — a rare, severe premature-aging disorder.

Zokinvy is also used to treat processing-deficient Progeroid Laminopathies — a related group of rare genetic conditions — specifically those caused by a heterozygous LMNA mutation with progerin-like protein buildup, or by homozygous or compound heterozygous ZMPSTE24 mutations. It is not indicated for other Progeroid Syndromes or processing-proficient Progeroid Laminopathies.
