Evening Primrose and Defibrotide Sodium

Here is the interaction we found between these two, with the evidence behind it.

ANTICOAGULANT/ANTIPLATELET DRUGS

Why this applies to Defibrotide Sodium

Defibrotide Sodium falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Evening Primrose interacts with.

Theoretically, evening primrose oil may increase the risk of bleeding if used with anticoagulant or antiplatelet drugs.
Evening primrose oil contains gamma linolenic acid (GLA). There is preliminary clinical evidence that GLA can reduce platelet aggregation and prolong bleeding time.

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist: defibrotide (Defitelio) is a powerful clot-preventing medicine given in the hospital after a stem-cell transplant, specifically for a serious liver condition. Because it already lowers your body's tendency to form clots, anything else that thins the blood can stack on top of it. Evening primrose oil contains GLA, which lab and early studies suggest may make platelets a little less sticky and slightly lengthen bleeding time.

Put together, the worry is a theoretical bump in bleeding risk, think easy bruising, nosebleeds, or bleeding that's harder to stop. Honestly, since defibrotide is only given under close hospital supervision, your care team is already watching this carefully. Still, tell your doctor or pharmacist about any supplements so they can make the call.

From our pharmacist’s overview on the Defibrotide Sodium page

“Defibrotide (Defitelio) is an intravenous drug for severe veno-occlusive disease of the liver after a stem cell transplant, and part of how it works is by reducing the blood's tendency to clot. That makes bleeding the single thread running through this entire list, and it deserves real care.”

Evening primrose oil is a seed oil rich in gamma-linolenic acid (GLA), an omega-6 fatty acid, that is popularly used for skin conditions, PMS, and breast pain. The evidence behind most of these uses is mixed or weak, and it is not proven to be a reliable treatment for any condition. It is generally well tolerated for short-term use, but talk to your pharmacist or doctor before starting it, especially if you are pregnant or have a bleeding disorder.
