Evening Primrose and Fluoxetine

Here is the interaction we found between these two, with the evidence behind it.

Minor

CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES

Why this applies to Fluoxetine

Fluoxetine falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Evening Primrose interacts with.

Theoretically, evening primrose may increase the levels and clinical effects of CYP2C9 substrates.
In vitro research shows that linoleic acid, a constituent of evening primrose oil, inhibits CYP2C9.
Severity Minor Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist if you take Prozac (fluoxetine) alongside evening primrose oil. There's a lab-only finding that linoleic acid in evening primrose may block an enzyme called CYP2C9, which is one of the liver pathways that helps clear certain drugs. In theory, that could let a little more fluoxetine linger in your system, slightly nudging up effects like nausea, jitteriness, trouble sleeping, headache, or dry mouth.

I want to be honest about how soft this is. It's theoretical, based only on test-tube data, and rated minor. Fluoxetine is mostly handled by other enzymes too, so a real-world problem is unlikely. Still, if you notice new or stronger side effects after adding it, give your pharmacist or doctor a quick call before changing anything.

From our pharmacist’s overview on the Fluoxetine page

“Grapefruit is the surprise at the top of fluoxetine's (Prozac) supplement list. In large amounts it can stretch the QT interval, the electrical pause between heartbeats, and fluoxetine, a widely prescribed antidepressant, can do the same, raising the odds of a dangerous rhythm.”

About Evening Primrose

What is Evening Primrose?

Evening Primrose
  • Eczema and skin health
  • PMS symptoms
  • Breast pain (mastalgia)
  • Menopause symptoms
  • Inflammation
  • Diabetic nerve pain

Evening primrose oil is a seed oil rich in gamma-linolenic acid (GLA), an omega-6 fatty acid, that is popularly used for skin conditions, PMS, and breast pain. The evidence behind most of these uses is mixed or weak, and it is not proven to be a reliable treatment for any condition. It is generally well tolerated for short-term use, but talk to your pharmacist or doctor before starting it, especially if you are pregnant or have a bleeding disorder.
