Limonene + Hyoscyamine, Phenobarbital
Hyoscyamine, Phenobarbital brand names: Levsin Pb, Levsinex Pb Timecaps.
Hyoscyamine, Phenobarbital falls into 2 drug categories that Limonene can interact with; the most serious is rated moderate. There's preliminary evidence that limonene might be a substrate for cytochrome P450 2C19 (CYP2C19).

These two interact in 2 distinct ways. Each one is explained separately below.
Why Are There 2 Interactions?
Limonene interacts with whole categories of medication. Hyoscyamine, Phenobarbital falls into 2 of those categories, so it can interact with Limonene in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).
CYTOCHROME P450 2C19 (CYP2C19) INDUCERS
1 of 2Why this applies to Hyoscyamine, Phenobarbital
Hyoscyamine, Phenobarbital falls under CYTOCHROME P450 2C19 (CYP2C19) INDUCERS — the group Limonene interacts with.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the thing to know about this pairing: it's really about what the medicine might do to the limonene, not the other way around. Your combo product contains phenobarbital, which is a classic enzyme booster (it speeds up several liver pathways, including CYP2C19). Since limonene appears to be broken down by that same pathway, taking it alongside phenobarbital could clear the limonene faster and make it less effective for whatever you're hoping it helps with.
I want to be honest about how shaky this is. The evidence comes only from lab and animal work, and it has never actually been reported in people. Food amounts of citrus won't matter at all. The bigger conversation is your phenobarbital itself, since it's a sedating, habit-forming drug. Please don't change anything without checking with your pharmacist or doctor first.

CYTOCHROME P450 2C9 (CYP2C9) INDUCERS
2 of 2Hyoscyamine, Phenobarbital also falls under CYTOCHROME P450 2C9 (CYP2C9) INDUCERS.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the thing to keep in mind with this pairing: the interaction runs in the opposite direction from what most people expect. It's not that your Levsin PB changes, it's that the medicine may change the supplement. The phenobarbital in this combo is a known enzyme booster (it revs up liver pathways including CYP2C9), and limonene appears to be broken down partly by that same pathway. So phenobarbital could speed up how fast your body clears limonene, meaning you may simply get less benefit from the supplement.
I want to be honest about the evidence here, though. This is based only on lab and animal data, so it's a soft, theoretical concern rather than something well proven in people. Food amounts of citrus won't matter. If you're considering a concentrated limonene supplement, just run it by your pharmacist or doctor first.
