Here's the gist with kava and warfarin. Your liver uses an enzyme called CYP2C19 (among others) to help clear warfarin out of your system. Lab studies suggest kava can block that enzyme, which could let warfarin build up a little. Since warfarin is a narrow-therapeutic-index drug, meaning the gap between too little and too much is small, even modest changes can matter. More warfarin effect could mean easier bruising, nosebleeds, or more serious bleeding.

That said, this is so far only seen in test-tube research, not in people, so it's theoretical. On top of that, kava carries its own risk of liver damage. I wouldn't pair them without checking in. Please talk with your pharmacist or doctor before adding kava, and keep up with your INR blood tests.

Here's the gist for warfarin. Your liver uses an enzyme called CYP2C9 to clear warfarin out of your system. With a drug as finely tuned as warfarin, even small shifts matter, so that could mean easy bruising, nosebleeds, bleeding gums, or more serious bleeding.

I want to be honest about the evidence, though. This effect has only been seen in test tubes, not in real people, so it's theoretical at this point. On top of that, kava itself has been tied to rare liver harm, which is its own concern. Please don't start or stop anything on your own. It is unclear if kava inhibits CYP1A2; research is conflicting.
Although in vitro research and a case report suggest that kava inhibits CYP1A2, more robust clinical evidence shows that kava has no effect on CYP1A2. In a clinical study in healthy volunteers, taking kava 1000 mg twice daily (containing a daily dose of 138 mg kavalactones) for 28 days had no effect on CYP1A2 activity.

Here's the honest picture with kava and warfarin. The concern on paper is that kava might slow down a liver enzyme called CYP1A2, which is one of the pathways your body uses to clear warfarin. If that enzyme were blocked, warfarin could build up, and warfarin is a blood thinner with a very narrow margin. Too much can mean easy bruising, nosebleeds, or more serious bleeding.

That said, the better-quality human research found kava did not actually change CYP1A2 activity, so this interaction is rated minor and unlikely. The bigger real-world worry is that kava can rarely harm the liver, and your liver is what processes warfarin. Please loop in your pharmacist or doctor before combining them and keep up with your INR checks.

Warfarin is a finicky drug where small shifts matter, so don't start kava on your own. Loop in your pharmacist or doctor first, especially since kava itself can stress the liver, which warfarin also relies on.

What Is Warfarin Used For?

Brand names: Coumadin, Panwarfin, Sofarin

Warfarin sodium tablets and Jantoven tablets are both used to prevent and treat venous thrombosis (blood clots in the veins) and pulmonary embolism (a clot in the lungs). They are also used to prevent clot-related complications in people with atrial fibrillation (an irregular heart rhythm) or who have had a heart valve replaced.

In people who have had a heart attack, warfarin is used to reduce the risk of death, another heart attack, stroke, or other clotting events. It does not dissolve an existing clot, but it helps prevent the clot from growing and stops new dangerous clots from forming.
