Losartan Potassium falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Kava interacts with.

Theoretically, kava might increase levels of CYP2C9 substrates.
In vitro research shows that kava significantly inhibits CYP2C9 enzymes. This effect has not yet been reported in humans.

Here's a wrinkle worth knowing about. Losartan is actually a prodrug, meaning your body has to convert it (using a liver enzyme called CYP2C9) into its more active form to get the full blood-pressure-lowering benefit. Lab studies suggest kava can slow that same enzyme down. In theory, that could mean less of losartan gets activated, so it may not work quite as well for your blood pressure.

I want to be honest about how soft this is: this comes from test-tube research only and hasn't actually been seen in real people, so it's more of a maybe than a proven problem. Still, since kava also carries its own liver risk, it's worth a quick chat with your pharmacist or doctor before combining the two. Don't change your losartan on your own.

It is unclear if kava inhibits CYP3AA; research is conflicting.
Although in vitro research suggests that kava inhibits CYP3A4, more robust clinical evidence shows that kava has no effect on CYP3A4. In a clinical study in healthy volunteers, taking kava 1000 mg twice daily (containing a daily dose of 138 mg kavalactones) for 28 days had no effect on CYP3A4 activity.

Here's the short version: this pairing of kava and losartan (Cozaar) is very unlikely to cause a real problem. The concern on file is about an enzyme called CYP3A4, which your body uses to help process losartan. Lab studies once hinted that kava might slow this enzyme down, but a better human study (people taking kava for 28 days) showed no meaningful effect on it.

So in plain terms, kava isn't expected to noticeably change how losartan controls your blood pressure or to ramp up its usual effects like dizziness or low blood pressure. My bigger caution with kava is actually its rare link to liver injury, so please check with your pharmacist or doctor before starting it, especially if you have any liver concerns.

Kava is a Pacific Island plant traditionally used to promote relaxation and ease anxiety, and some studies suggest it may help mild anxiety. However, kava has been linked to rare but serious liver damage, so it should be used with caution, avoided by people with liver problems, and only used after talking with your pharmacist or doctor.

About Losartan

What Is Losartan Used For?

Brand name: Cozaar

Losartan tablets — available as branded Cozaar and several generic products — are used to treat high blood pressure (hypertension) in adults and children 6 years and older. Lowering blood pressure reduces the risk of strokes and heart attacks. Losartan tablets are also used to reduce the risk of stroke in people with high blood pressure who also have left ventricular hypertrophy (an enlarged left side of the heart). Note: evidence suggests this stroke-risk benefit may not apply to Black patients.

Losartan tablets are additionally approved to protect the kidneys in people with type 2 diabetes, high blood pressure, and signs of kidney damage (diabetic nephropathy). Arbli, the oral suspension form, shares these same approved uses.
