Warfarin falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Kava interacts with.
In vitro research shows that kava significantly inhibits CYP2C19 enzymes. This effect has not yet been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for warfarin. Lab studies suggest kava may slow down an enzyme called CYP2C19, which is one of the pathways your body uses to clear warfarin. If that happens, warfarin could build up a bit and thin your blood more than intended. With a drug as finely tuned as warfarin, even small shifts matter, and too much thinning can show up as easy bruising, nosebleeds, bleeding gums, or blood in the urine or stool.
Now, the reassuring part: this is only seen in test-tube research so far, not in real people, so it's theoretical. On top of that, kava can stress the liver (where warfarin is processed), which is another reason for caution. Please don't start or stop anything on your own. Check with your pharmacist or doctor first, and they may simply watch your INR more closely.

Here's the situation with kava and warfarin. Your liver uses an enzyme called CYP2C9 to break warfarin down and clear it from your body. Lab studies suggest kava can block that enzyme. If that happened in people, warfarin could build up and thin your blood more than intended, which raises the risk of bruising and bleeding (think nosebleeds, bleeding gums, blood in urine or stool, or cuts that won't stop).
I want to be honest about the evidence, though: this comes from test-tube research only and hasn't actually been confirmed in humans. Still, warfarin is a finicky drug where small shifts matter, so I wouldn't add kava on your own. Please check with your pharmacist or doctor first, especially since kava can also stress the liver.

Although in vitro research and a case report suggest that kava inhibits CYP1A2, more robust clinical evidence shows that kava has no effect on CYP1A2. In a clinical study in healthy volunteers, taking kava 1000 mg twice daily (containing a daily dose of 138 mg kavalactones) for 28 days had no effect on CYP1A2 activity.
Interaction has been demonstrated in animal or in lab research but has been shown not to occur in humans..
Here's the honest picture with kava and warfarin. The theory is that kava might slow down a liver enzyme called CYP1A2, which plays a small part in clearing warfarin. If that were true, warfarin could build up and thin your blood more than intended, raising the risk of bruising or bleeding. But the better-quality research doesn't support this. In one study, healthy volunteers took kava for 28 days and saw no real change in that enzyme's activity, so this interaction is rated minor and unlikely.
That said, warfarin is a narrow-therapeutic-index drug, meaning small shifts matter. My bigger concern with kava is its rare link to liver harm, since your liver helps process warfarin. Please loop in your pharmacist or doctor before combining them.

Although in vitro research suggests that kava inhibits CYP3A4, more robust clinical evidence shows that kava has no effect on CYP3A4. In a clinical study in healthy volunteers, taking kava 1000 mg twice daily (containing a daily dose of 138 mg kavalactones) for 28 days had no effect on CYP3A4 activity.

Here's the good news on this one: even though kava was once thought to slow down a liver enzyme called CYP3A4 (which plays a part in clearing warfarin), the better human research doesn't back that up. In a study where healthy people took kava for 28 days, it didn't actually change that enzyme's activity. So a strong, direct kava-warfarin clash through this pathway looks unlikely.
That said, warfarin is a finicky blood thinner where even small shifts can mean bleeding (bruising, nosebleeds) or, on the flip side, clotting. So I'd still loop in your pharmacist or doctor before adding kava. The bigger concern with kava is its rare risk of liver harm, and your liver matters for warfarin too.

What Is Warfarin Used For?
Brand name: Warfarin
Warfarin sodium tablets and Jantoven tablets are both used to prevent and treat venous thrombosis (blood clots in the veins) and pulmonary embolism (a clot in the lungs). They are also used to prevent clot-related complications in people with atrial fibrillation (an irregular heart rhythm) or who have had a heart valve replaced.
In people who have had a heart attack, warfarin is used to reduce the risk of death, another heart attack, stroke, or other clotting events. It does not dissolve an existing clot, but it helps prevent the clot from growing and stops new dangerous clots from forming.
