Hydrocodone, Ibuprofen falls under CNS DEPRESSANTS — the group Kava interacts with.

Combining kava with CNS depressants can have additive sedative effects.
Kava has CNS depressant effects. Concomitant use of kava with other CNS depressants can increase the risk of drowsiness and motor reflex depression. Clinical practice guidelines from a joint taskforce of the World Federation of Societies of Biological Psychiatry (WFSBP) and the Canadian Network for Mood and Anxiety Treatments (CANMAT) recommend that CNS depressants, including alcohol and benzodiazepines, not be used with kava.

Vicoprofen pairs hydrocodone, an opioid pain reliever, with ibuprofen. The piece that matters here is the hydrocodone, since it already slows down the brain and can make you sleepy. Kava works in a similar calming way, so taking the two together stacks one sedative on top of another.

The real concern is added drowsiness, foggy thinking, slowed reflexes, and in a worst case, slowed or shallow breathing. This is a well-supported interaction, and major safety guidelines specifically warn against mixing kava with sedating medicines. On top of that, both kava and hydrocodone/ibuprofen can be hard on the liver, which is another reason to be careful. Please talk with your pharmacist or doctor before using them together.

Theoretically, kava might increase levels of CYP2C9 substrates.
In vitro research shows that kava significantly inhibits CYP2C9 enzymes. This effect has not yet been reported in humans.

Vicoprofen pairs hydrocodone (an opioid pain reliever) with ibuprofen (an anti-inflammatory). The ibuprofen part is partly cleared by a liver enzyme called CYP2C9. In test-tube studies, kava seems to slow that enzyme down, which in theory could let ibuprofen levels drift a little higher. If that happened, you might notice more of ibuprofen's usual issues, like stomach upset, heartburn, or, with heavier use, a higher risk of stomach bleeding or kidney strain.

Keep in mind this is only lab data, not something confirmed in real people, so I wouldn't lose sleep over it. The bigger worry honestly is that both kava and the hydrocodone side can make you drowsy, and both can be tough on the liver. Please run this combo by your pharmacist or doctor first.

It is unclear if kava inhibits CYP1A2; research is conflicting.
In vitro research shows that kava extract significantly inhibits CYP2D6. However, clinical research shows that kava does not affect the metabolism of CYP2D6 substrates in humans.

Here's the short version on combining kava with Vicoprofen (which pairs hydrocodone with ibuprofen). The hydrocodone part is actually a prodrug of sorts. Your liver enzyme CYP2D6 converts some of it into hydromorphone, a stronger painkiller. In test-tube studies, kava looked like it might block CYP2D6, which in theory could lower that conversion and slightly dull pain relief. But here's the reassuring part: actual human studies showed kava did not meaningfully change how CYP2D6 drugs are handled.

So this is mostly a theoretical concern with low-quality evidence behind it. My bigger real-world worry is that both kava and hydrocodone can make you drowsy and sleepy, and kava can stress the liver. Please run this combo by your pharmacist or doctor first.

It is unclear if kava inhibits CYP3AA; research is conflicting.
Although in vitro research suggests that kava inhibits CYP3A4, more robust clinical evidence shows that kava has no effect on CYP3A4. In a clinical study in healthy volunteers, taking kava 1000 mg twice daily (containing a daily dose of 138 mg kavalactones) for 28 days had no effect on CYP3A4 activity.

Vicoprofen combines hydrocodone (a narcotic pain reliever) with ibuprofen. The concern on file is whether kava slows down CYP3A4, one of the liver enzymes that helps break down hydrocodone. If it did, you might in theory see a bit more of the opioid's effects, like extra drowsiness, dizziness, constipation, or slowed breathing.

Here's the reassuring part: this interaction is rated minor and unlikely. Better-quality human research showed kava had no real effect on CYP3A4, so the lab worries didn't hold up in people. That said, kava is itself a sedative, so stacking it with an opioid could leave you groggier than expected. Kava can also strain the liver. Please check with your pharmacist or doctor before combining them.
