Esomeprazole, Naproxen falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Kava interacts with.

Theoretically, kava might increase levels of CYP2C19 substrates.
In vitro research shows that kava significantly inhibits CYP2C19 enzymes. This effect has not yet been reported in humans.

Vimovo combines two medicines: esomeprazole (a stomach-acid blocker) and naproxen (an anti-inflammatory pain reliever). The esomeprazole part is cleared from your body partly by a liver enzyme called CYP2C19. In lab tests (test tubes, not real people), kava seems to slow this enzyme down. If that happened in your body, esomeprazole could build up a bit, though for a heartburn medicine that usually just means slightly more acid-blocking, not dangerous effects.

Here's my honest take: this is a theoretical concern only, based on lab studies, and it hasn't been seen in actual patients. The bigger thing I'd flag is that both kava and naproxen can stress the liver, so combining them isn't ideal. Please check with your pharmacist or doctor before pairing them.

Vimovo combines two medicines: naproxen (an anti-inflammatory pain reliever) and esomeprazole (which protects your stomach). The naproxen part is partly cleared by a liver enzyme called CYP2C9. In lab studies, kava appears to slow that enzyme down. If that happened in your body, naproxen could build up a bit, which might mean more of its usual side effects, like stomach upset, heartburn, fluid retention, or, with heavier use, a higher risk of stomach irritation or kidney strain.

I want to be honest about the strength of this: it has only been seen in test-tube research, not in real people, so it's theoretical for now. There's also a separate reason to be cautious, both kava and naproxen can stress the liver. Please check with your pharmacist or doctor before combining them.

Esomeprazole, Naproxen also falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES.

It is unclear if kava inhibits CYP1A2; research is conflicting.
Although in vitro research and a case report suggest that kava inhibits CYP1A2, more robust clinical evidence shows that kava has no effect on CYP1A2. In a clinical study in healthy volunteers, taking kava 1000 mg twice daily (containing a daily dose of 138 mg kavalactones) for 28 days had no effect on CYP1A2 activity.

Here's the good news on combining kava with Vimovo (which is esomeprazole paired with naproxen). The worry on file is whether kava slows down a liver enzyme called CYP1A2 that helps process certain drugs. But the better human research settles this: kava did not meaningfully change CYP1A2 activity, so this particular interaction is rated minor and unlikely. Neither esomeprazole nor naproxen is a prodrug, so we're not flipping any direction here.

That said, I'd raise a different flag. Kava has been tied to rare but serious liver injury, and naproxen can also stress the liver and stomach. Stacking liver risks isn't ideal. Please check with your pharmacist or doctor before pairing these, especially if you have any liver concerns.
