Why this applies to Donepezil

Donepezil falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Kava interacts with.

It is unclear if kava inhibits CYP1A2; research is conflicting.
In vitro research shows that kava extract significantly inhibits CYP2D6. However, clinical research shows that kava does not affect the metabolism of CYP2D6 substrates in humans.

Here's the gist for donepezil (Aricept) and kava. Donepezil is partly broken down by a liver enzyme called CYP2D6. In a test tube, kava seems to block that enzyme, which in theory could let donepezil build up and bring out more of its usual side effects, things like nausea, diarrhea, a slower heartbeat, dizziness, or trouble sleeping. The reassuring part is that actual studies in people did not show kava meaningfully changing how the body clears CYP2D6 drugs, so this concern stays mostly theoretical and the real-world risk looks low.

My bigger worry with kava is its rare link to serious liver damage, which is worth discussing on its own. Before combining the two, please check with your pharmacist or doctor rather than starting or stopping anything yourself.

Donepezil also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

It is unclear if kava inhibits CYP3AA; research is conflicting.
Although in vitro research suggests that kava inhibits CYP3A4, more robust clinical evidence shows that kava has no effect on CYP3A4. In a clinical study in healthy volunteers, taking kava 1000 mg twice daily (containing a daily dose of 138 mg kavalactones) for 28 days had no effect on CYP3A4 activity.

Here's the short version on combining kava with donepezil (Aricept). Donepezil is partly broken down by a liver enzyme called CYP3A4. Early lab-dish studies hinted that kava might slow that enzyme down, which in theory could let donepezil build up a bit and bring on more of its usual side effects, things like nausea, diarrhea, a slower heart rate, dizziness, or trouble sleeping.

The good news: when researchers actually tested this in people, kava didn't meaningfully change CYP3A4 activity. So this concern stays mostly theoretical and is rated minor and unlikely. My bigger caution with kava is its rare link to liver damage. Since donepezil is for someone with dementia, please run any kava use past their pharmacist or doctor first.
