Here's the thing to know about pairing St. John's wort with donepezil: this herb revs up a liver enzyme called CYP3A4, which is one of the main ways your body breaks donepezil down. When that enzyme works faster, donepezil gets cleared out more quickly, so the level in your blood can drop. Donepezil is not a prodrug, so this means you may simply get less benefit from it.

In real life that could look like the memory, attention, or daily-function support slipping back, which is hard to notice gradually in someone with Alzheimer's. Concentrated supplement doses (hyperforin around 1 mg or more) are where this matters most. Please don't stop either one on your own. Just check with your pharmacist or doctor first.
