Why this applies to Riociguat

Riociguat falls under P-GLYCOPROTEIN SUBSTRATES — the group Kava interacts with.

It is unclear if kava inhibits P-glycoprotein (P-gp); research is conflicting.
In vitro research shows that kava can inhibit P-gp efflux. However, a clinical study in healthy volunteers shows that taking kava standardized to provide 225 mg kavalactones daily for 14 days does not affect the pharmacokinetics of digoxin, a P-gp substrate. It is possible that the use of other P-gp substrates or higher doses of kava might still inhibit P-gp.

Here's the gist with riociguat (Adempas) and kava. Riociguat is moved around your body in part by a transporter called P-glycoprotein, or P-gp. In a test tube, kava can slow that transporter down, which in theory could let a bit more riociguat build up. Since riociguat lowers blood pressure, more of it could mean more low blood pressure, dizziness, lightheadedness, headache, or feeling faint.

The reassuring part: this is mostly lab-dish data. In a real human study, kava did not change levels of digoxin, another P-gp drug. So a true interaction here is uncertain, especially at culinary amounts versus stronger supplement doses. Kava can also stress the liver, so please check with your pharmacist or doctor before combining the two.

Riociguat also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

It is unclear if kava inhibits CYP3AA; research is conflicting.
Although in vitro research suggests that kava inhibits CYP3A4, more robust clinical evidence shows that kava has no effect on CYP3A4. In a clinical study in healthy volunteers, taking kava 1000 mg twice daily (containing a daily dose of 138 mg kavalactones) for 28 days had no effect on CYP3A4 activity.

Here's the gist for Adempas (riociguat) and kava. Riociguat is partly cleared by a liver enzyme called CYP3A4. The theory was that kava might slow that enzyme down, which could let riociguat build up and lead to more low blood pressure (think dizziness, lightheadedness, or fainting), or more headaches and swelling. But that worry comes mostly from test-tube studies.

The better human research is reassuring: in healthy volunteers, kava didn't actually change CYP3A4 activity. So a real-world interaction here looks unlikely and minor. That said, kava can rarely harm the liver, and your liver helps process riociguat, so it's still worth a quick check-in with your pharmacist or doctor before combining them.

About Riociguat

What Is Riociguat Used For?

Brand name: Adempas

Riociguat tablets — sold as Adempas and under the generic name Riociguat — are used to treat adults with two types of pulmonary hypertension (high blood pressure in the lungs).

The first is chronic thromboembolic pulmonary hypertension (CTEPH), a condition caused by blood clots in the lung arteries, either after surgery or when surgery is not an option. The second is pulmonary arterial hypertension (PAH), where the arteries carrying blood to the lungs are narrowed or blocked. In both conditions, riociguat helps improve exercise ability and functional status, and in PAH it also helps slow the disease from getting worse.
