Theoretically, using kava with hepatotoxic drugs might increase the risk of liver damage.
Kava has been linked with over 100 cases of hepatotoxicity. Most cases occur with excessive and prolonged use. There is some concern that kava can adversely affect the liver, especially when used in combination with hepatotoxic drugs.

Here's the concern with pairing kava and ripretinib (Qinlock): both can be tough on your liver. Ripretinib already carries a known risk of raising liver enzymes and, less commonly, causing more serious liver injury, which is why your oncology team checks your bloodwork during treatment. Kava has been tied to over 100 reports of liver damage, mostly with heavy or long-term use. Stacking the two could, in theory, add to that strain.

To be fair, this is based on case reports and general caution rather than strong studies, so it's a 'play it safe' situation, not a proven danger. Still, with a serious cancer medicine like this, I'd hold off on kava and check with your oncologist or pharmacist first. Watch for yellowing skin or eyes, dark urine, belly pain, or unusual tiredness.

Ripretinib also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

It is unclear if kava inhibits CYP3AA; research is conflicting.
Although in vitro research suggests that kava inhibits CYP3A4, more robust clinical evidence shows that kava has no effect on CYP3A4. In a clinical study in healthy volunteers, taking kava 1000 mg twice daily (containing a daily dose of 138 mg kavalactones) for 28 days had no effect on CYP3A4 activity.

Here's the short version: your body uses a liver enzyme called CYP3A4 to help clear ripretinib (Qinlock). On paper, kava was once thought to slow that enzyme down, which in theory could let ripretinib build up and cause more side effects like fatigue, hair loss, muscle aches, or high blood pressure. But the better human research tells a calmer story. In a study of healthy people, daily kava for 28 days didn't actually change CYP3A4 activity, so this concern stays mostly theoretical and the overall risk is rated minor and unlikely.

What I'd flag instead is your liver. Kava has been tied to rare but serious liver injury, and ripretinib is already monitored for liver effects in cancer care. Please loop in your oncology team or pharmacist before adding kava, just to be safe.
