Here's the concern with combining BMPEA and donepezil. Your body relies partly on an enzyme called CYP2D6 to clear donepezil out of your system. Lab studies suggest BMPEA may block that enzyme. If that happens, donepezil could build up to higher levels than usual, which can mean stronger effects and more side effects. With donepezil, that often shows up as nausea, diarrhea, loss of appetite, dizziness, a slow heartbeat, or trouble sleeping.

I want to keep this in perspective though. This warning is theoretical, based on test-tube research, not real people. On top of that, BMPEA is a hidden, unapproved stimulant that can raise heart rate and blood pressure, so I'd steer clear of it regardless. Please run any supplement past your pharmacist or doctor first.

Here's the gist for donepezil (Aricept). Your body partly clears this medicine using a liver enzyme called CYP3A4. In lab tests, BMPEA appears to slow that enzyme down a bit. In theory, that could let donepezil build up a little higher than usual, which might nudge up its typical side effects, things like nausea, diarrhea, slow heart rate, dizziness, or trouble sleeping.

I want to be honest about how soft this evidence is. It comes only from test-tube research, not from people, so we don't really know if it matters in real life. That said, BMPEA is a hidden amphetamine-like stimulant with real heart concerns, and I'd steer clear of it for that reason alone. Please run any supplement past your pharmacist or doctor first.
