CNS DEPRESSANTS

Buprenorphine/Naloxone falls under CNS DEPRESSANTS — the group Kava interacts with.
Kava has CNS depressant effects. Concomitant use of kava with other CNS depressants can increase the risk of drowsiness and motor reflex depression. Clinical practice guidelines from a joint taskforce of the World Federation of Societies of Biological Psychiatry (WFSBP) and the Canadian Network for Mood and Anxiety Treatments (CANMAT) recommend that CNS depressants, including alcohol and benzodiazepines, not be used with kava.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the simple version: both kava and Suboxone calm down your nervous system, and stacking two calming agents can pile their effects on top of each other. The buprenorphine in Suboxone already slows breathing and causes drowsiness, and kava's kavalactones seem to work on those same quiet-the-brain pathways. Together, you could feel unusually sleepy, foggy, dizzy, or slow to react, and in a worst case your breathing could be more depressed than expected.
This is a well-supported concern, not just theory, which is why expert guidelines warn against mixing kava with sedating medicines. There's also a separate liver worry, since both can stress the liver. Please don't stop anything on your own, but do talk with your pharmacist or doctor before combining these.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
2 of 2Buprenorphine/Naloxone also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.
Although in vitro research suggests that kava inhibits CYP3A4, more robust clinical evidence shows that kava has no effect on CYP3A4. In a clinical study in healthy volunteers, taking kava 1000 mg twice daily (containing a daily dose of 138 mg kavalactones) for 28 days had no effect on CYP3A4 activity.
Interaction has been demonstrated in animal or in lab research but has been shown not to occur in humans..
Here's the short version for Suboxone and kava. The buprenorphine part is partly cleared by a liver enzyme called CYP3A4. In a test tube, kava looked like it might block that enzyme, which in theory could let buprenorphine build up and add to sleepiness, slowed breathing, or feeling foggy. But the better human study tells a calmer story: people taking kava daily for a month showed no real change in CYP3A4 activity. So this particular interaction is rated minor and unlikely.
My bigger concern is overlap, not metabolism. Both kava and Suboxone can be sedating and both can stress the liver, so combining them may add to drowsiness. Please run it by your pharmacist or prescriber before mixing them.
