Donepezil falls under CHOLINERGIC DRUGS — the group Bael interacts with.

Bael leaf extract shows acetylcholinesterase (AChE) inhibitory activity in vitro. Theoretically, bael might have additive effects with cholinergic drugs and increase the risk of cholinergic side effects.
Cholinergic drugs include bethanechol (Urecholine), donepezil (Aricept), echothiophate (Phospholine Iodide), edrophonium (Enlon, Reversol, Tensilon), neostigmine (Prostigmin), physostigmine (Antilirium), pyridostigmine (Mestinon, Regonol), succinylcholine (Anectine, Quelicin), and tacrine (Cognex).

Here's the simple version: donepezil works by blocking an enzyme called acetylcholinesterase, which raises a brain chemical that helps with memory and thinking. Lab studies suggest bael leaf extract may block that same enzyme a little. So the worry is that the two together could push in the same direction, giving you a bit too much cholinergic activity.

If that happened, you might notice more of donepezil's usual side effects, things like nausea, diarrhea, stomach cramps, a slower heartbeat, more frequent urination, or muscle twitching. I want to be honest though: this is only based on test-tube work, not real people, so it's theoretical. Food amounts of bael are unlikely to matter much, but concentrated supplements are less predictable. Please check with your pharmacist or doctor before combining them.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Donepezil also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Bael and its constituents marmelosin and marmesinin inhibited cytochrome P450 3A4 (CYP3A4) activity in vitro. So far, this interaction has not been reported in humans. Theoretically, bael might increase levels of drugs metabolized by CYP3A4.
Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others. Use bael cautiously or avoid in patients taking these drugs.

Here's the gist for your Alzheimer's medicine. Your body partly clears donepezil using a liver enzyme called CYP3A4. In lab dishes, compounds in bael appear to slow that enzyme down. If that same effect happened in people, donepezil could clear a little more slowly and build up slightly, which might nudge up its usual side effects like nausea, diarrhea, loss of appetite, dizziness, vivid dreams, or a slower heart rate.

I want to keep this in perspective, though. This is only a test-tube finding, it has never actually been reported in real patients, and it's rated minor. Donepezil is not a prodrug, so the direction is more drug, not less. Eating bael as a food is unlikely to matter much. Still, before starting concentrated bael supplements, run it by your pharmacist or doctor.
