Why this applies to Nifedipine

Nifedipine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Bael interacts with.

Bael and its constituents marmelosin and marmesinin inhibited cytochrome P450 3A4 (CYP3A4) activity in vitro. So far, this interaction has not been reported in humans. Theoretically, bael might increase levels of drugs metabolized by CYP3A4.
Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others. Use bael cautiously or avoid in patients taking these drugs.

Here's the gist for nifedipine and bael. Your body relies on an enzyme called CYP3A4 to break nifedipine down. In lab studies (test tube only), compounds in bael appeared to slow that enzyme. If that happened in real life, nifedipine could clear more slowly and build up a bit, which might exaggerate its usual effects. For nifedipine, that could mean a dip in blood pressure that feels like dizziness, flushing, headache, swollen ankles, or a faster heartbeat.

I want to keep this in perspective, though. This is only a theoretical, lab-based concern, and it hasn't been seen in people. Eating bael as a fruit is unlikely to matter; concentrated supplements are the bigger question mark. Just run it by your pharmacist or doctor before adding a bael supplement.

What Is Nifedipine Used For?

Nifedipine treats high blood pressure (hypertension), chronic stable angina (chest pain brought on by physical effort), and vasospastic angina (chest pain caused by spasm of the coronary arteries, sometimes called Prinzmetal's angina). Lowering blood pressure with nifedipine helps reduce the risk of strokes and heart attacks.

The capsule form is approved for vasospastic angina and chronic stable angina only — it is not approved for high blood pressure. The extended-release tablet (including Procardia) covers all three uses: vasospastic angina, chronic stable angina, and high blood pressure.
