Kava has CNS depressant effects. Concomitant use of kava with other CNS depressants can increase the risk of drowsiness and motor reflex depression. Clinical practice guidelines from a joint taskforce of the World Federation of Societies of Biological Psychiatry (WFSBP) and the Canadian Network for Mood and Anxiety Treatments (CANMAT) recommend that CNS depressants, including alcohol and benzodiazepines, not be used with kava.

The piece of this combination pill that matters most here is the hydrocodone, an opioid pain reliever that already slows down your brain and body. Kava has its own calming, sedative-type effects, so pairing the two can stack on top of each other. That means more drowsiness, dizziness, slowed breathing, and slower reflexes than you'd expect from the pain medicine alone. This is a well-supported concern, which is why expert groups advise against mixing kava with sedating medicines.

In real life, that could look like feeling extra groggy, foggy, or unsteady, and it makes activities like driving riskier. One more note: both kava and the aspirin in this product can stress the liver, so that's worth keeping in mind too. Please don't change anything on your own, just check with your pharmacist or doctor first.

Although in vitro research and a case report suggest that kava inhibits CYP1A2, more robust clinical evidence shows that kava has no effect on CYP1A2. In a clinical study in healthy volunteers, taking kava 1000 mg twice daily (containing a daily dose of 138 mg kavalactones) for 28 days had no effect on CYP1A2 activity.

Here's the practical takeaway for this combo pill (which mixes aspirin, caffeine, and the opioid hydrocodone). The interaction on file centers on whether kava slows down a liver enzyme called CYP1A2, which mainly handles the caffeine portion. Honestly, the evidence is shaky and conflicting. A solid human study found kava had no real effect on CYP1A2, so any caffeine buildup (jitters, racing heart, trouble sleeping) is unlikely.

What I'd watch more closely is the overlap in calming effects. Kava can make you drowsy, and so can hydrocodone. Together they may pile on sedation. There's also a shared liver concern, since both kava and aspirin can stress the liver. Please run this past your pharmacist or doctor before combining them.

This combination pill (Damason-P) pairs aspirin and caffeine with hydrocodone, an opioid pain reliever. The hydrocodone part is partly handled by a liver enzyme called CYP2D6, which helps convert it into a more active form. In test-tube studies, kava looked like it might slow that enzyme down. If that actually happened in people, you could in theory get slightly less pain relief from the hydrocodone, or shifted side effects.

Here's the reassuring part: actual human studies show kava does not meaningfully change how CYP2D6 drugs are processed. So this is mostly a lab-only, low-likelihood concern. I'd worry far more about the real overlap, both kava and hydrocodone can cause drowsiness, and both can stress the liver (as can aspirin in high doses). Please check with your pharmacist or doctor before combining them.

Here's the good news about combining kava with this combo pill (which contains the opioid hydrocodone, aspirin, and caffeine): the enzyme angle people worry about, CYP3A4, doesn't seem to be a real problem. Early lab dish studies hinted kava might slow CYP3A4, which could theoretically let hydrocodone build up. But a better human study showed kava had no meaningful effect on that enzyme, so this particular interaction is rated minor and unlikely.

The thing I'd actually keep an eye on is overlap in calming effects. Both kava and hydrocodone can make you drowsy or foggy, and together that could add up. Kava has also been tied to rare liver injury, which matters when you're already taking aspirin. Please check with your pharmacist or doctor before pairing these.
